Patients should receive mechanical thrombectomy In selected patients with AIS within 6 to 16 hours
with a stent retriever if they meet all the following of last known normal who have LVO in the anterior
criteria: circulation and meet other DAWN or DEFUSE 3
(1) prestroke mRS score of 0 to 1 eligibility criteria, mechanical thrombectomy is
(2) causative occlusion of the internal carotid recommended. (I,A)
artery or MCA segment 1 (M1)
(3) age ≥18 years
(4) NIHSS score of ≥6
(5) ASPECTS of ≥6 and

• 4. Etiology - TOAST
• A 23-year-old lady presented with generalized tonic-clonic seizures.
She had a history of febrile convulsion when she was a child. Her mot
her noticed in these 10 years she has had intermittent loss of awarene
ss with abnormal hand movement lasting 13 minutes which episodes
are triggered by sleep-deprivation. She sometimes feels that the envir
onment surrounding her seems familiar, even if she visits the places fo
r the first time.

Temporal lobe epilepsy
• Temporal lobe epilepsy
• Lateral TLE:
• Mesial TLE: more common (2/3 cases)
• Hippocampal sclerosis – secondary febrile convulsion
• Other benign or malignant tumor, viral and other infection and
parasite disease, CVD, developmental malformation, trauma.
How to choose AEDs
• Seizure type
• Comorbidities
• AEDs → comorbid disease
• Previous drugs → seizure threshold
• Comorbid disease → AED toxicity
• Drug-drug interactions
• Side effect of AEDs
• Pharmacodynamics & pharmacokinetics
Antiepileptic drug selection
Monotherapy era
Equally or higher efficacious than polytherapy
Better tolerated
No drug interaction
Possibly better compliance
Better cost effective
Particularly desirable in
- women
- elderly
- patients with co-morbid conditions
Pregnancy with epilepsy
• If seizure uncontrolled. Should beware pregnancy.
• More comorbid
