1. Concept KSGM

MPT and KSGM will bring in some part of Health & Safety Procedure to
Myanmar which are conducted in Japan.

This elevates few levels of standard comparing to the current situation in

Myanmar.

The intention for conducting these actions are ;

1. to protect human life of Myanmar workers who are working for JO Project.
2. to contribute to raise the standard of Myanmar telecom related construction,
especially regarding to Health & Safety.

(2) Duty of Each Position

a. KSGM HSM = Health & Safety Manager of KSGM
b. MPT HSM = Health & Safety Manager of MPT
c. prime constructor HSM = Health & Safety Manager of prime contractor
HSM is responsible for reporting daily ,weekly and monthly progress to KSGM and MPT.
If there are anything to decide outside the KSGM Health & Safety Specifications, HSM will
discuss with KSGM HSM and decide what to do.

d. Sub con HSE = Sub contractor HSE
HSE is responsible for supervising the SSV and making daily & weekly reports to RM.
e. SSV = Subcontractor Super Visor
SSV will take control and supervise responsible construction.
who must visit and check all of their on going work area at least twice a day before and after
the lunch break for example.
f. When HSE should leave the site during construction, prime constructor should assign person
behalf of HSE.

3. Urgent Reporting Procedure (2) Reporting Rule KSGM
1st priority is taking care of injured person or recover the existing service. 1 st action carried out
properly then report immediately with following rules. Followings are included “Incident”
a. Any accidents or “close calls” .
b. Someone gets sick and need urgent medical treatment.
c. Damage to any Equipment for construction and existing.
d. Damage for MPT’s property, 3rd party’s property and public property.
e. Contaminant to environment such as river water, soil and so on.
f. Injury to Human.
Firstly, contact to the KSGM and/or MPT by the phone.(see Appendix 2)
Secondly, Send e-mail with follow information.( template Appendix 3)
g. Date and Time of the Incident.
h. Place of the Incident
i. Situation of the Incident
j. Cause of the Incident
k. When injured or sick, please also include the following items;
(1) person name and age
(2) company name
(3) part of body that was injured, degree of injury.
First report is important, even if all of the information is not available.

 4. Recovery Procedure KSGM

(1) Reporting

a. Reporting Incident
Reporting of incident is done according the Urgent Reporting Procedure.
KSGM HSM will determine to stop the project with following Criteria.

b. Stop the project completely when the following event occurs,

Fire and/ or explosion on/close the construction
Fumes or gases (noxious gases/ vapors)
Injury
- Death of person
- Loss of limb(s)
- Injuries with “serious” blood loss
c. Stop the work on site
Small fire around site. Smoke development.
Gasoline/Diesel spilled on site
Injury - Small cuts or wounds
- Fainting of person

4. Recovery Procedure KSGM

(3) Action Meeting by KSGM & prime constructor

Urgently open a meeting and determine the preventive actions
　　 The reporting format “Action Meeting Report” is Appendix 4.

(4) Confirmation & Approval of Action

KSGM HSM reviews & approves “Action Meeting 　 Report”.

(5) Implement action plan to subcontractors by prime constructor.

KSGM HSM reviews & approves “Action plan” implementation.

(6) Restart Project by KSGM HSM

KSGM notice to prime constructor regarding “restart the project”

 5. Safety Management Procedure KSGM

(1) Construction Plan

Before starting any activity at site, Construction Plan must be submitted from
prime constructor to KSGM. (see Appendix 5)
(2) Accident Prediction Everyday, before starting any activity at site, Accident
Prediction must be conducted at the site using Accident Prediction Format.
Procedure and it’s format is Appendix 6.
(3) Safety Equipment Check
Everyday before starting any activity at site, prime constructor HSM and sub
contractor HSM will inspect safety Equipment and report the check result to
KSGM. (See report format Appendix 7)
(4) Health Check
Everyday before starting any activity at site, prime constructor HSM and Sub
contractor HSM should conduct Health check for all workers and report to
KSGM. Also, HSM should check drinking water & sunshade.
(See Health Check Format Appendix 8)
(5) Insurance
prime constructor must have valid Overall Construction Work Insurance.

5. Safety Management Procedure KSGM

(6) ID Card All people working at site must wear ID -card with info, company
name, full name of the person, photo of the person, blood type, contact number of
Vendor.

(7) Others Other issues relating to Safety Management Procedure is stated on the
Health and Safety Specifications which is on Appendix1. Vendor must follow this
Health and Safety Specification at all the construction relating to KSGM.

6. Precautions KSGM
(1) MPT & KSGM forbid the labor of;
a. Child Labor (ages under 18 years old)
b. Slavery or Forced Labor.

(2) Caution Signs The caution signs has to be displayed for every site before
starting any construction.
a. Signs have to have text in Myanmar and English Language.
b. Minimum List of sign to be used;
High voltage warning, First Aid box, No entry, PPE and site security, Think
safety slogan, Fire extinguisher, Drink Water sign, Deep excavation, Danger
Construction site no entry, Low Headroom, Site code and Location, Emergency
numbers.
