Maprotiline Hydrochloride

Mechanism of action
It strongly inhibits the reuptake of nor-adrenaline in the
brain and peripheral tissues.

Maprotiline also exerts a sedative effect on the anxiety


component of depressive illness.

Pharmacodynamics

1. Shares a number of basic therapeutic properties with the


tricyclics

2. Displays a well-balanced spectrum of action

3. Has a potent and selective inhibitory effect on the reuptake of


noradrenaline into the presynaptic neurones

4. Brightens the patient’s mood and alleviates anxiety, agitation,


and psychomotor retardation

Peak Plasma Concentrations : Within 8 hrs.

Protein Binding : 88-89 %

Plasma Elimination Half Life : 43 hr.

Excretion : Within 21 days. Through

Indications

Depression:
 All forms of depression
 Endogenous: involutional form
 Psychogenic: reactive, neurotic, exhaustion
 Somatogenic : organic and symptomatic
 Masked depression
 Menopausal depression
 Other depressive mood disorders characterized by anxiety,
dysphoria, or irritability

Masked depression

When no organic cause is found & patients do not respond


well to specific therapy

Patients complaints are like:

 Sleep disturbance

 Loss of interest

 Headache

 GI disturbance

 Poor concentration

Ludiomil –the best alternative because of its


anxiety-relieving property

Why Ludiomil?
Anxilolytic cannot improve the underlying depressive mood
and do not posses any depression relieving effect.
 Ludiomil the only single drug having anxiety-relieving
antidepressant with a bipolar effect
They tend to induce dependence from which is it is extremely
difficult to free the patients
 Ludiomil does not
Since depressive states are often associated with a risk of
suicide, its necessary above all to brighten the patients mood.
Tranquillizers have no influence on suicidal tendencies
 Ludiomil brightens the patients mood
SSRIs cause sexual Dysfunction
 Ludiomil does not have this.

Dosages:
Dosage Guidelines:
• Always individualized

• Start with low dosage

• Gradually be increased cautiously

• Usually one daily dose

• A daily dose of 150 mg. should not be exceeded

• Patients must be kept under close surveillance

Mild to moderate depression

25 mg. 1-3 times daily or 25-75 mg. once a day

Severe cases : 25 mg. t.i.d. or 75 mg. once a day. If necessary

raise upto 150 mg. maximum

Other depressive mood disorders

Children and adolescents

Start with 10 mg. t.i.d. or 25 mg. once a day

Can be raised stepwise to upto 25 mg. t.i.d.

Elderly

Same as children and adolescents

Contraindications

Known hyper-sensitivity to maprotiline

Should not be employed in epileptics

In patients with freshly sustained myocardial infarction

In patients with defects in narrow angle glaucoma, or


retention of urine
