I, EDMUND LOYOLA GAITOS, also known as Edmund Cabugayan, of legal
age, Filipino citizen, single and a resident of Brgy. 95, Caiba-an, Tacloban City, after
having been duly sworn to in accordance with law, hereby depose and state: THAT--1. I was born out of wedlock on January 16, 1986 in Tacloban City of the couple
PRAXEDES LOYOLA GAITOS and CARLOS ESPERAS CABUGAYAN
(deceased), who cohabited with each other without the benefit of marriage;
2. I was baptized in Sto. Nino Parish Church, Tacloban City one year later as
EDMUND CABUGAYAN;
3. Since grade school up to the present, I have used the name EDMUND
CABUGAYAN, and was popularly known by said name;
4. However, my parents were not able to register my live birth before the Civil
Registry of Tacloban City;
5. Recently, when I went to the Civil Registrar of Tacloban City to apply for late
registration of my live birth, I was informed that since I was an illegitimate child, I
should be carrying only the name of my mother (GAITOS).
6. With that information, I decided to change my name from EDMUND
CABUGAYAN to EDMUND LOYOLA GAITOS; as such, these two names actually
refer to one and the same person as the herein affiant.
7. I am executing this affidavit to attest to the foregoing facts in order to clarify,
correct or modify my personal and official records; further, this affidavit serves to
substantiate my application to change my name in the baptismal record from EDMUND
CABUGAYAN to EDMUND LOYOLA GAITOS as provided by the Family Code,
notwithstanding the Revilla Law.
