    Subject: - APPLICATION FOR RETIRMENT ON MEDICAL GROUNDS

    Sir, It is stated that I was appointed as Sub-Engineer on 22/10/1975 with employee code No 00295 and presently working in BPS-17 in Building Directorate LDA. I have almost completed more then 34 years of service at my credit. On 25/03/09 I suffer from Stroke on my right side since then dont have ability to perform my duties, right now I am not in position to continue my service there fore apply to get retirement on medical grounds. It is there fore, requested that kindly be allowed retirement on medical grounds w.e.f 24/12/2009(A/N) along with all dues & oblige. I will be thankful for an early action in this regard.
