    REQUEST FOR DONATION

    Dear Dato/YB/Sir/Madam,

    I Falvius Eba @ Flavius Yabah Mau ( IC NO: 520212-13-5409 ), am writing this letter
    to inform that my wife Rose Yupak Anak Dami ( IC NO: 570728-13-5528 ) has
    recently been diagnosed with a Prolapsed Interverbtebral Disk which causes to be
    operated as soon as possible. What burdens our family is the cost of our travel
    expenses from Limbang to Kuching, accommodation and daily expenses. As the
    hospital informed my family that my wife's healing process would take about a month
    or so and this would also involve increasing the cost of my family expenses there later.
    As I am short of money and financial problem I am humbly hoping that you will be able
    to provide financial assistance to reduce the burden I am facing.

    With this letter, It is my great hope that you can help reduce the burden that I have.
    With this opportunity, I thank you for your effort to consider and solved my financial
    problems. Attached herewith photocopy letter from the Miri Hospital for your
    information.
