To Whom This May Concern

I am writing this letter to request for my child Fai Shaira N. Dela Cruz to please be excused
from your respective class in grade 8 section Aristotle, on the 19th of June until the 20th, year
2018 so she can attend her grandmother’s funeral.
I ask that if you would please give her any notes that she may miss, and any assignments,
activities, quizzes etc., that will be handed out so she does not fall behind. Thank you so much
for your time and understanding.
Attached herewith is the proof of my identification for verification purposes.
