I. SCOPE OF WORK

The contractor, ASB Enterprises has completed the above Project in accordance with the approved
Specifications, Schedules, Variation and/or Extra Works and other related Contract Documents.

II. CERTIFICATION

The Contractor certifies that the Scope of Work involving the furnishing of all necessary labor,
supervision, tools and equipment has been fully completed.

The Contractor, likewise, agrees to correct any workmanship defect accruing after the turn-over of the
project within the guarantee period, effective duration of which shall be 1 YEAR/365 Days
commencing from the date of Completion and Acceptance is signed by the owner.

III. RECOMMENDATION

Cognizant of the full completion by the Contractor of the Scope of Work, we hereby recommend the
acceptance of the Project described in Items I and II.

IV. ACCEPTANCE

The Owner, Globe Telecom, hereby accepts the Project and assumes full possession of the same.
