APPLICATION TO GRADUATE
(UNDERGRADUATE STUDENTS ONLY – INCLUDING STUDENTS AT DLSU LAGUNA CAMPUS)
AY 2018-19, Term 3
To apply to graduate, please follow the steps below:
STEP 1: (20 May to 13 Jun ‘19) CHECK YOUR CURRICULUM AUDIT
Please check your Curriculum Audit (through MLS):
Make sure to check all the items. If you completed a course which has a different course
code, please process a Course Crediting Form. Students with below 25 units remaining
in the curriculum audit can proceed with the next step.

STEP 2: (17 Jun to 13 Jul ‘19) ACCOMPLISH EXIT SURVEY Note : For International Students, you are required
to accomplish a separate Exit Survey which

Visit http://my.dlsu.edu.ph Mondays to Fridays (0800-1900) Saturdays (0800-1200) 22 to 27 Jul 2019 – COB
Click ‘Apply to Graduate’.
A confirmation e-mail will be sent to the e-mail address provided in the online ATG. Please
check your e-mail to confirm that your application had been successful.
22 to 27 Jul 2019 – SOE
If no e-mail confirmation was received, please go to OUR immediately to report the problem.

STEP 3A: (22 Jul to 10 Aug ‘19) CORRECTION OF RECORDS

STEP 5: (05 to 27 Aug ‘19) RELEASE OF ASSESSMENT OF FEES 5 to 10 Aug 2019 – CCS
 An e-mail will be sent to the e-mail address provided by the student
during the online ATG
 Please follow the instruction in the e-mail.

IMPORTANT:
STEP 6: (05 to 27 Aug ‘19) PAYMENT OF FEES
(a) Pay the assessed amount not later than 27 August 2019 The name of the student will be included in the Tentative List
of Graduates that will be posted on 07 October 2019
(b) Please print the Schedule of Graduation Related activities through this link:

only when payment of the Graduation fees has been made.

Note : Students who applied to graduate during the previous term still need to process the online application to graduate (ATG)
this term. Please follow Steps 1-4 above. During reapplication, if a confirmation number appears even before accomplishing the
online ATG (Step 3), this means that your previous application had been successfully reactivated. For any question, please email
