SUBJECT : REQUEST FOR HOT WORK ACTIVITY

Hi Sir,

The Vessel MV Aqua Marine consigned to LA Filipina Uygonco Corp. with expected DOA of
August 30, 2019 with commodity of Urea Prilled.

In view of the bagging operation at HCPTI, may we request approval for Hot works activity/permit
from the HCPTI Safety Department which is required whenever there will be repairs and /or reconditioning
before and during the said operation.

We commit to deploy a firewatcher, fire extinguishers whenever there will be hot works. Likewise,
we shall require all employees of Seabar Terminal Inc. to wear appropriate personal protective equipment
(PPE) and to observe the safety policy of HCPTI.
