Introduction

An electrocardiogram is a picture of the electrical conduction

of the heart. By examining changes from normal on the ECG, can

identify a multitude of cardiac disease processes. There are two ways

to learn ECG interpretation pattern recognition and understanding

the exact electrical vectors recorded by an ECG as they relate to car-

diac electrophysiology and most people learn a combination of

both. Parts of an ECG The standard ECG has 12 leads. Six of the leads

are considered “limb leads” because they are placed on the arms

and/or legs of the individual. The other six leads are considered

“precordial leads” because they are placed on the torso

(precordium). The six limb leads are called lead I, II, III, aVL, aVR

and aVF. The letter “a” stands for “augmented,” as these leads are cal-

culated as a combination of leads I, II and III. The six precordial

leads are called leads V1, V2, V3, V4, V5 and V6.

Introduction

Cardiac function is driven by electricity. An electrical im-

pulse is generated in the sinoatrial node, and in the healthy

heart that impulse then travels in a precise direction through

myocardial tissue by way of specialized conduction pathways.

The pathways are critical to optimal function for a few reasons.

They ensure that the impulse is distributed simultaneously

throughout the contractile tissue of the atria and then the ventri-

cles. The 12 Leads A lead is the distance between 2 points, not the

electrode that is attached to the patient’s limbs or anterior chest

wall. Learning to read and interpret 12-lead ECGs successfully re-

lies heavily on grasping the concept that a lead is a viewpoint.
