Completed Task/s

June 1 – Dissemination of Enrolment Infomercial

- The infographics on how to enroll was disseminated by the teacher through Facebook
post and chat groups to inform students and parents on how to enroll this school year.
June 2 – Orientation on Learner Enrolment and Survey Form (LESF)
- The school conducted a virtual meeting regarding the Learner Enrolment and Survey
Form (LESF). The LIS Coordinator discussed the relevance and importance of LESF to
categorize the learners needed learning modality for this school year. It is very essential to us
because of the online teaching that will be conducted for this S.Y
June 3-11 – The teacher will communicate and contact their previous students to answer the
LESF to know if they want to continue their studies in our school or will stop for this
S.Y.
JUNE 4 – Attended the Live webinar of SDO-Caloocan CID entitled “Online Division Writeshop
on the Development of Self-Learning Modules (SLM) for all Learning Areas” and other
webinar.
June 5 – Enrolment monitoring; As of June 5, 2020, 13 of my students (7-Oman) have
accomplished the LESF.

Ongoing Task/s
- Monitoring for the remaining students who haven’t answered to the LESF.
- Attending different free webinars whether authorized or unauthorized by DepEd to gain
more learning in this New Normal and prepare for the Distance Learning Instruction.
- Consolidation of data (LESF)
- Follow-up learners status during LESF application.
Tasks for Next Week
June 8-11
- Contract tracing for the remaining students who haven’t answered to the LESF.
- Development of Contextualized Self-Learning Modules (SLMs) for all Learning Areas
- Attend webinars for future references during distance learning of learners and teachers.
