In the interest to public service, you are hereby assigned as I.C.T. DISTRICT COMMITTEE MEMBER of
Sugbongcogon District, Sugbongcogon, Misamis Oriental. This Designation Order shall be valid only effective
March 31, 2017 to March 31, 2018. This Order may be revoked before the termination date or as the need arises.

As I.C.T. DISTRICT Committee MEMBER, you are expected to do the following:

1. To support all the ICT activities in the district.

2. Assist the implementation of ICT Programs & Projects in the district.
3. Develop ICT – instructional tools/materials to be used in the district.
4. Facilitate in complying & submitting on-line monthly reports specifically the FORM 3 & others.
5. Does other related functions in the district.

This is in addition to but not part of your teaching load.
