A. Participant’s The following were provided as evaluative observation of the
Feedback participants:
1. Design of the Event
Portfolio Day is a synchronize activity intended to showcase
pupils outputs and performances. To inform the parents on how
their children work and behave in school, to let them continuously
support their children and address challenges immediately in
cooperation with teachers and other school personnel. And in the
same time early enrolment was also scheduled to gather qualified
learners for kinder and grade I.

2. Methodology
The date for portfolio day and early enrolment was scheduled
nationwide and all the participants were informed and instructed
to come to witness their children performances through their
outputs and academic achievements.
3. Management of the Activity
The collection of data was headed by the school heads and
master teachers and supported by the grade chairmen and other
teachers.

4. Resource Persons
Principal, Department Head, Master Teachers and others.

5. Audience Participation

102 stakeholders attended the general assembly and others

attended in homeroom meeting together with their children.

Synchronized Portfolio day and Early Registration scheduled on

January 26, 2019.

Cawayan Integrated School Elementary Department school heads

and teachers prepared designated areas for pupils outputs exhibit, a
hall where parents were gathered and an area for the Early
Registration. The activity was spearheaded by the school heads and
Master Teachers.

At 9:00 o’clock in the morning the first of the four parts of the
program hosted by Mrs. Hazel Paller, MT-I. It started with a prayer
led by Ms. Ofelia Espelita, MT-II and followed by the singing of the
Philippine National Anthem conducted by Mrs. Mila E. De Una, MT-II.
Then Mr. Esmeraldo B. Gomba , department head gave his Welcome
Remarks. After which was the intermission number of the selected
grade VI pupils. Mr. Noel A. Dolorzo the principal gave his message.

The second part was the GPTA General Assembly. When it

started Dr. Gaudencio C. Aljibe, the assistant schools division
superintendent came to monitor the activity. He was introduced to the
parents and was given time to have his message. When the division
monitoring team left the general assembly continued with Mrs.
Wenhelmina C. Bondoc, MT-I (Teachers Club Treasurer), and Alvin D.
Tejero ( GPTA President) presented the income raised during the

“ STAKEHOLDERS NIGHT.” Lastly the principal, Mr. Noel A. Dolorzo

informed the parents about the early registration of the Kindergarten
and Grade One, the age requirement and documents needed. He also
mentioned the upcoming activities in the month of February.

The third part of the activity was the Gallery Walk of the
parents to the Portfolio Display.

The last activity was the Homeroom PTA Meeting by class.
