Your firm, Majd Al Bina Contracting Company , has been found to

be in violation of your contract by non-compliance with applicable Regional, Local, or Company HSE
requirements.

On 4th November 2019 (date), in accordance with the Company Contractor HSE Adherence Policy, your
representative, Mohammad Sheraz / Site Civil Supervisor , was given a Notice of HSE Non-
Compliance (copy attached). This notice specifies areas where your company does not comply with Regional,
Local, or Company HSE requirements, and requests that these items be corrected immediately.

If they are not corrected, more stringent measures will be taken in accordance with Company Contractor HSE
Adherence Policy.
