STUDENTS’ SURVEY
It has been two weeks since we have started our Academic Year 2020-2021 and went fully
online in delivering our classes.
We are dedicated to offering you an excellent online learning experience. As a student taking an
online course this semester, your responses to the following questions will help us improve our
course design, our instruction, and the type of services we are able to offer our online students.
We are exceedingly interested in hearing about either your experiences with Flexible Learning or
your opinions about it to enhance its delivery mechanism assuring students’ success.
The primary objective of this survey is to assess the condition of enrolled students in order to
inform University’s direction on the implementation of flexible learning in the curriculum,
instructional development and other instructional approaches and interventions to mitigate the
effect of CoVid-19 in instructions.
Your answers will be treated with utmost confidentiality. This survey will not any personal
information. The university is committed to comply with RA 10173 known as the Philippine
Data Privacy Act of 2012in handling your responses with utmost confidentiality and anonymity.
We do not use any technical or nontechnical means of tracking individual responses to this
survey.
