PROBLEM/ ISSUES/ CONCERN

1.) Student’s tardiness.
2.) Absenteeism and tardiness.
3.) Behavioral problems of the students.
4.) Failing grades
5.) Lack of moral and financial support from Parents.
SOLUTIONS:
1.) Call the attention of the parents and guardians through sending the letter of
notification.
2.) Schedule and conduct home visitation.
3.) Follow-up student’s status twice a month.
4.) Teacher-student-parent conference.
5.) Update the parent’s/ guardians if their child is absent from the class.
6.) Meet the parent’s at least twice every quarter.
TARGET:
1.) To increase at least 5% of student attendance and punctuality.
2.) To counsel 75% of students with behavioral problems.
3.) To update learner’s about their academic status.
4.) To encourage parents/guardians to support their students morally and
financially.

RECCOMMENDATIONS:
1.) Set a schedule for home visitation.
2.) Conduct parent-teacher conference from time to time through technologies.
3.) Conduct counseling to student’s with behavioral problems.
4.) Talk to the students as often as possible to strengthen rapport towards them.
5.) Releasing of report card every grading period.
