Dear Vice Mayor Chiong,

The Philippine Charity Sweepstakes Office (PCSO) intends to donate to the City
Government a one (1) unit Nissan Van Patient Transport Vehicle (PTV) for the use
of the residents of this City.

With the fight of the City Government against the threat and effect of COVID-19,
the intended donation is of great use to the City.

Hence, this is to respectfully request the honorable body for the passage of a
Resolution authorizing the undersigned to accept the donation and for the
authority of the same to sign the Deed of Donation, herein attached.
