1.0 INTRODUCTION

Obza Services Sdn Bhd (hereinafter called the Obza) has been awarded by Lynas Malaysia Sdn Bhd (hereinafter
called the Lynas) to execute cleaning work using High Pressure Water Jetting Machine with capacity 20,000 Psi
to the Spiral Heat Exchanger (Alfa Larva).

2.0 PURPOSE

This document envisages the work completion report for cleaning work using High Pressure Water Jetting
Machine with capacity 20,000 Psi to the Spiral Heat Exchanger (Alfa Larva). Tag number for heat exchanger is
2E2109B.

3.0 SCOPE OF WORK

Scopes of work for this task is to clean Spiral Heat Exchanger (Alfa Larva) using High Pressure Water Jetting
Machine with capacity 20,000 Psi. Location of cleaning work is at AMC Workshop.

4.0 PURCHASE ORDER

Purchase order for this project received on Sept 23rd 2020 through email from Lynas Procurement Department.
The details of the Purchase Order is as follow: -

11.0 BLASTING AND PAINTING WORK

No blasting work during this Shut Down campaign. Manual painting work using roller have been applied.

12.0 SAFETY ISSUE AND REPORT

No major issue reported and recorded. Minor issue highlighted during the execution of the job are as follow: -

a. Supervisor required to monitor at all-time activity at site

b. All workers compulsory to attend Morning Tool Box Talk every morning to ensure all information received
by all workers.
c. All workers compulsory to fill and sign attendance every day prior start work.
d. Complete PPE at all time during working at site
e. Time keeping
f. Housekeeping after work done every day.
