MYRNA C. CABOTAJE, MD, MPH, CESO III, in her capacity as the

Regional Director of the aforementioned office, and hereinafter
referred to as “DOH RO I”;

and

The 783 AKSYON RADYO, with office address of Pagdalagan Norte, San
Fernando, La Union 2500, represented herein by its Manager, WALTER
DAVID B. ABUAN, JR., herein referred to as “783 AKSYON RADYO”.

WHEREAS, Department of Health (DOH), as the government’s over-all

technical authority on health is responsible for ensuring access to basic public
health services by all Filipinos through the provision of quality health care and
the regulation of all health services and products;

WHEREAS, the DOH Regional Office I (DOH RO I) understand the

importance of multimedia partners such as radio station to promote programs of
DOH as well as to educate the general public on Health and Nutrition issues

specifically families in Region I

WHEREAS, DOH- Regional Office I is need of a radio station to carry out

the multimedia campaign for Health and Nutrition program of the agency;

WHEREAS, the Regional Bids and Awards Committee declared 783

AKSYON RADYO as the lowest (offer/charged) and responsive radio station to
carry out the multimedia campaign for Health and Nutrition program of the
agency.

WHEREAS, DOHRO I recognizes the capability and reach of 783

AKSYON RADYO, as the leading local AM radio station in the Province of La

Union, in creating awareness and appreciation in relevant advocacy programs for

health and nutrition;

NOW THEREOF, for and in consideration of the foregoing premises

hereinafter stipulated, the parties have agreed as follows:

I. Responsibilities of the Parties:

a. Provide the Health Program Plugs and Infomercials to be aired;

b. Designate a focal person who will coordinate with 783 AKSYON
RADYO staff regarding the requirements of the projects’

d. Provide technical assistance as may needed; and
e. Issues a certificate of availability of funds; and process payment
upon receipt of the required outputs.;

B. The 783 AKSYON RADYO shall:

1. Airing of Health Programs Plugs and infomercials, at three (3)

times per day, 22 days per month (Monday to Friday), 30 seconds
on prime-time affairs program or a total of sixty - six (66) time
tags per month or a total of six hundred sixty (726) time tag
exposures starting March 1, 2018 to December 31, 2018.

completion, financial reports and a CD of the recorded Health
Programs and infomercials for archiving and future use of the
region.

II. Terms/Manner of Payment

The total media package value amounts to One Hundred Thousand

Ninety Eight PesosTwo Hundred Seventeen Thousand (₱217198, 8000.00),
inclusive of VAT chargeable against the LHSDA Funds of DOH RO I
which shall be paid as follows:

The DOH RO I reserves the right to cancel the contract due to

unavailability of funds.

III. Termination and Amendments

This Contract shall be automatically terminated upon the expiration of the

term or until December 31, 2018 unless pre-terminated by agreement of the parties.

Amendments of this contract may be validly be made upon agreement of the

parties and should be made before the expiration of the term of the contract.
