Consequence:
Potential delay of sailaway transportation set from loadout yard
to field. CONTRACTOR had to use another towing tug, Magellan 1
who was on standby for another barge at THHE Yard, Port
Kelang.

Why:
1) The vessel received commitment from another charterer
2) Change of offshore installation schedule results to the delay of
vessel onhire date
3) No official confirmation from COMPANY on the acceptance of
some relaxations of technical criteria

Learnt Asia Installation Delay on anchor operations at F23R-A & D12DR-A and ensure sufficient spares are available onboard.

Consequence:
Longer operation and fatigue for AHT1 to run anchor alone.

Why:
1) AHT2 (David Tide II) brokedown and late client approval
(OVID) due to slow response from owner of replacement AHT
(Miclyn Venture) .
2) Custom clearance of spare parts couldnt be done on weekend
(to be confirmed by Marine)

braces. jacket leg).

Consequence:
1) Minimal clearance between the tubing with tie down
attachment. This requires extreme care when cutting the
tiedown braces to avoid damage to the tubings which could
jeopardise the grout packer and pile gripper activation.

Why:
1) The tube routing design does not consider tiedown location.

25 Lesson UIA - SMEP LADR-A Jacket Operation & L(3) 1 Issue: • Use mechanical clamp for long pile upending
Learnt Asia Installation Installation Grout packer punctured and failed to be fully inflated (3 out of 4
pile sleeves). • Grind flush the upending trunnion after flame cut
if upending the pile using trunnions.
Consequence:
Delay (additional 1.5 days) in overall grouting operation due to
the need to set grout plug at pile sleeve A2, B1 and B2 as means
of grout seal .

Why:
Based on COMPANY's investigation and complimented by
Pronal's investigation findings, the grout packer failure was
neither due to product defect nor damage during installation at
yard. This is QC'ed with factory acceptance test and yard function
test. The 6mm pile upending trunnion residual protrusion and
sharp edges on the surface of the trunnion after flame cut could
have punctured the grout packer.

A number of facilities were damaged when lowering down the • Better weight estimation with realistic
heavy lift slings due to swell experienced that resulted the crane allowances. Use auxiliary block if possible that

excessive movement.
Why:
1) Discrepancy of LADR-A topside actual recorded lift weight
(630MT) and estimated lift weight (750MT) . Too much of
allowances were applied on the weight that gives an over-
estimated weight resulting to the used of Main Hook.
2) Not sufficient protection on the main deck for the equipments.
3) Close proximity between the heavy lift slings and
equipment/pipings on the topside main deck. Potential clashes
were identified during rig up of the slings. However, swinging of
the lifting slings after topside set down (sling in slacked
condition) was overlooked.

3) Onshore testing of the system was not conducted at yard
using the offshore equipment
4) Jacket upending operation was delayed due to wrong
compartment flooded. Subsequently, it was found that the
connection on the HPU was wrongly connected by vendor.
