PATIENT DETAILS
Mother: Sally Andres, 36 y/o, G2P2 (had 2 pregnancies and 2 deliveries)
Spouse: Frank Andres – Construction Worker
Youngest baby: Madeline, 3 ½ months old.
Remarks from the Scenario:
 This is her first time to go to the health center since vaccines are more expensive in
the private clinic
 Infant’s previous vaccines were given at the hospital upon birth
 Mother asks the nurse about the side effects, different vaccines to be given after today’s
doses
 Nurse will advise the mother to bring back the baby on her 10 th week of age (after 2
weeks) for the 2nd dose of Penta 5 and PCV
