Nx 1 = Maon – OPV Vaccine Nx 5 = Sanchez
Nx 2 = Rivera – Pentavalent 2 Documentation / Orientation to the next
vaccination / Reminders
Nx 3 = Loyola – PCV Conjugate
Nx 6 = Ramos
Introduction to the Vaccines
Nx 4 = Mesa
PATIENT DETAILS
Mother: Sally Andres, 36 y/o, G2P2 (had 2 pregnancies and 2 deliveries)
Spouse: Frank Andres – Construction Worker
Youngest baby: Madeline, 3 ½ months old.
Remarks from the Scenario:
 This is her first time to go to the health center since vaccines are more expensive in
the private clinic
 Infant’s previous vaccines were given at the hospital upon birth
 Mother asks the nurse about the side effects, different vaccines to be given after today’s
doses
 Nurse will advise the mother to bring back the baby on her 10 th week of age (after 2
weeks) for the 2nd dose of Penta 5 and PCV

FLOW
ROLE SEQUENCE
Mother – Maon/Someone in 1. Mother enters the clinic
the house who’s willing and was quickly guided by
Nurse 2 and 3 – Loyola, Nurse Mara to sit down and
Rivera write her name on a piece of
Doctor – Mesa paper/record sheet.
2. After a while Nurse Kyla
will call out her baby’s name
and ask to go to the room.
3. Dra. Mesa will introduce
herself, welcome her to the
health clinic and ask
necessary questions to the
mother.
4. After the discussion Dra.
Mesa will introduce her to
Nurse Aubrey who’ll take the
Physical Assessment of the
baby
Nurse 5 – Sanchez 1. Nurse Aubrey will take the
Nurse 6 – Ramos necessary Vital Signs and PA
Baby – Doll for the baby. Nurse Louwella
will help her document the
necessary PA.
2. Nurse Aubrey will then
guide her to Nurse Mesa
Nurse 4 – Mesa 1. Nurse Mesa will explain
Mother – Rivera the purpose of the vaccine
and entertain the mother’s
questions regarding the
vaccine.
2. After question are
entertained. Nurse Mesa will
ask her to go the room where
REMARKS
If feasible, we can change
the virtual background of the
video call in a health clinic
setting.
e.g. “Ilang taon na po si
baby?” “nag-pa-vaccine na
po ba ng (say the prior
vaccines before reaching 3
months old)” the mother will
answer with: The infant’s
previous vaccines were given
at the hospital upon birth.
“Ilang taon na po yung una
niyong anak?” (if the gap is
low, advise family planning
maybe)
Outside the scene:
Maon will change into scrubs
for the next sequence
Rivera will change into the
costume of the mother while
baby is being examined (if
applicable)
Highlight question of the
mother: Side effects and
different vaccines to be given
after today’s doses

vaccination will take place
and ask her to wait patiently
as other patients are there
too.
Nurse 1/Mother – Maon 1. Nurse Loyola again ask
Nurse 2/Mother – Rivera the mother (Rivera) to sit
Nurse 3/Mother – Loyola down and wait for her baby’s
name to be called.
2. Administration of vaccines
is demonstrated while mother
is holding the baby.
3. After vaccination is
complete introduce mother
and baby to Nurse Louwella.
Nurse 6 – Ramos 1. Nurse Louwella will ask the Highlight of the scene: The
Mother - Maon mother of her name and
baby’s name on a logbook
and record the time and date 10th week of age (after 2
along with her signature.
2. Nurse will also provide an
improvised card to show
when is the date to
comeback for the second
dose of injection for the baby.
3. The mother will thank the
nurses and staffs and leave
the health clinic smiling.
NOTHING FOLLOWS
Option 1: The nurses will
take turns holding the baby
as the mother. Until Maon
will be the last one holding
the baby. Costume is not
necessary anymore
Option 2: Ask someone from
the house to be the one
holding the doll
Don’t forget to introduce
yourself and introduce the
vaccine that is being
administered to the mother
before injecting it to the baby.
Also, use the return
demonstration checklist
regarding parenteral
administration
Assessment and
documentation is still
necessary. Have an
improvised MAR with you to
document the date and time
when the vaccine was
administered.
