Mother: Sally Andres, 36 y/o, G2P2 (had 2 pregnancies and 2 deliveries)
Spouse: Frank Andres – Construction Worker
Youngest baby: Madeline, 3 ½ months old.
Remarks from the Scenario:
This is her first time to go to the health center since vaccines are more expensive in
the private clinic
Infant’s previous vaccines were given at the hospital upon birth
Mother asks the nurse about the side effects, different vaccines to be given after today’s
doses
Nurse will advise the mother to bring back the baby on her 10 th week of age (after 2
weeks) for the 2nd dose of Penta 5 and PCV
