Contact the LDM Program Management Team of your division (SGOD) /
region (NEAP-R). Divisions/Regions have varying systems of collecting
forms, and submission deadlines.
To download the document, go to FILE, then click on DOWNLOAD AS
PDF/EXCEL/PPT. You may also hover your cursor to the file thumbnail,
then click on the down arrow icon. You don’t need to send a request to
access/edit the file.
No, just submit your accomplished old evaluation form to the SDO/RO LDM
PMT. But you MAY also use the new version of the form should you decide
to redo the evaluation.
If the evaluator used the new version, reflect the ratings in Form 4 you will
submit. Do not include the ratings yet in the summary if the evaluator used
the old version. Submit both the old and new versions of the evaluation forms
to the link NEAP-CO will give you. Refer to the form at bit.ly/ldm1form3 .
