Individualized IEP Requirements

Student 1
Goals:
★ Attention and Time on Task - By October 20, 2021, in order to be competitively
employed after graduating from high school, after initiating a task, Student 1 will follow
through with the task to completion and turn in her finished product to have no more
than one missing assignment per quarter in each of her classes, to be measured by
grade reports and teacher observations
★ Math Calculation - By October 20, 2021, in order to be successful with college level
academics, Student 1 will perform the necessary calculations to achieve 80% accuracy on
assignments and assessments in her general education math class, to be measured by in
class assignments and assessments, and grade reports
Accommodations:
● Extended time on tests
● Extended time on assignments
● Visual cues from steps in problem solving, graphics, and models
● Calculation devices
● Providing sample problems for reference
● Using written notes, outlines, and instructions to aid in maintaining writing focus per
student request
● Graphic organizers to help her connect claims with evidence
● Short simple directions with examples
● Repeating directions
● Text highlighting
● Gaining attention before speaking
● Have student repeat or explain directions to check for understanding
● Explicitly give time limits for shorter assignments

Student 2
Goals:
★ Self advocacy - In order for Student 2 to work as a welder, he will need to learn how to
self advocate for himself in 3 out of 4 classes as measured by teacher feedback and
grade reports
Accommodations:
● Extended time on test and quizzes if requested by teacher or student
● Ability to take tests and quizzes in alternative setting upon request

Student 3
Goals:
★ Behavior Management - In order for Student 3 to work in the auto industry, he will be
able to identify anger cues, and use appropriate coping skills 80% of the time in
stressful situations to control his emotions by May 20, 202.
Accommodations:
● Prompts and redirection to stay focused
● Checks for understanding
● Extra time on assignments and tests (no more than one day late on assignments)
● Taking test and quizzes in alternative settings
● Copy of teacher notes upon request (only if he attempts to take notes)
● Ability to take short movement breaks

Student 4
Goals:
★ Organization and Work Completion -Career - Student 4 will work on improving his
organization skills by organizing his school work and having homework completed and
turned in on time in his classes in 3 out of 4 situations per month
★ Written Expression - In order to effectively communicate in writing as a student in a
vocational school and as a restaurant business owner, Student 4 will be able to write to a
given prompt with 3 paragraphs, meeting the writing rubric guideline with a 3 on a 4
point rubric
Accommodations:
● Extra time for writing & reading assignments ( 1-2 days) and assessments (time and a
half)
● Provide copies of notes
● Provide word banks, sight word lists
● Use of graphic organizers
● Allow oral response and verbal processing to set up his essays
● Use of technology for long written assignments
● Short breaks allowed to process anger, frustration, or anxiety as needed
● Access to audio books when appropriate
● Chunk writing assignments
● Allow Student 4 to perform the "self-talk" strategy while performing tasks
● Shorten writing assignments as needed
● Allowed to take tests over several days when a lot of writing is involved
● Writing rubrics will not include spelling as part of the grading criteria
● Allow to take tests in an alternate, quiet setting upon request

Student 5
Goals:
★ Mathematical calculation and problem solving - Student 5 will improve her math calculation
and problem-solving skills as evidenced by her averaging at least 70% average on her
math class assessments when using a calculator and self-prepared notes sheet allowed
on the assessments.
★ Expressive, receptive and pragmatic language
Accommodations:
● Visual supports/cues along with auditory
● Chunk lengthier assignments such as projects into parts with due dates
● Allow for short movement breaks in and out of the classroom
● Allow for use of stress ball or fidget in the classroom
● Extended time on assignments and tests (time and one half)
● Reading assistance on assessments unless reading itself is being assessed
● Allowed to read out-loud in an environment minimal of distractions
● Use of graphic organizers when writing essays
● Allow use of notes/study guides on tests
● Checks for understanding
● Provide copies of class notes to supplement
● Student 5’s own notes, upon her request
● Allow Student 5 to complete an assessment in an alternate setting outside the general
education classroom upon her request
● Allowed the use of a calculator when performing math problems

Student 6
Goals:
★ Organization and Self Initiation - By December 9, 2021, in order to be successful
working as a certified nursing assistant, Student 6 will improve her organizational skills by
checking in with her teachers when she misses class, and making up missing work in
order to complete the assignments required to pass, and receive credit for her classes
to be measured by teacher observations and grade checks.
★ Math calculation - By December 9, 2021, in order to be prepared to be successful
completing the training program to become a certified nursing assistant, Student 6 will
complete the necessary math calculations to correctly solve math problems she
encounters in her classes with 80% accuracy as measured by her classroom
assessments and curriculum based measurements.
Accommodations:
● Extra time to complete assignments and tests
● Allow extra time to process new information
● Organization strategies
● Repetition of directions Preferential seating
● Teacher Check in
● Use of a calculator, unless calculation is being tested
● Breaking larger assignments into smaller chunks

● Opportunities to work with special education staff when confused about how to
complete assignment

Student 7
Goals:
★ Math Calculation - In order to be better prepared for college level math, Student 7 will be
able to use mathematical practices to help make sense of the real-world by applying
geometric and algebraic equations to real-life situations with 80% accuracy
★ Organization & Work Completion - Student 7 will work on improving his organization skills by
organizing his school work and having homework completed and turned in on time in his
classes, with no more than one assignment missing in each quarter, in 3 out of 4
opportunities
Accommodations:
● Assistance with organizational skills as needed
● Test in a quiet, separate environment (resource room) to minimize distractions and talk
out questions
● Extended time for assignments and assessments as needed
● Shortened writing assignments as needed
● Access to graphic organizers for writing assignments
● Access to word prediction or speech to text programs for extended writing
● Access to audio books
● Allow the use of a calculator
● Provide copies of guided notes (fill in the blank)
● Allow for word banks on tests
● Oral presentation of assessments upon request
● Allow use of notes for assessments
