DESCRIPTION OF INSPECTION
The heat exchanger serial number S9740-01 was subjected to the following test [ as per procedure ppe/wi/07] which
was witnessed;
1]Hydro-static test tube side of the heat exchanger at 205 bar for a minimum of 60 minutes, as per ITP section 6.5
The above noted test was noted acceptable with no loss of pressure or visible leakage noted
A hydro test certificate was produced, reviewed and noted as acceptable. The test certificate was endorsed with the
Velosi Europe stamp, dated and signed off
The ITP section 6.7 states that another Hydro-static test, shell side of the heat exchanger at 29 bar with shell cover
fitted is also to be completed, but this heat exchanger design does not have a shell cover. Poole process stated that
the ITP is at code 1 final, but this could not be verified. So the extra Hydro-static test, shell side of the heat exchanger
at 29 bar with shell cover fitted was not completed, as there is no shell cover to fit
No damage was noted and the cast / heat numbers of the new RTJ flanges were noted and will be cross checked to
the material certificates as soon as the MDB is ready to review
CONCLUSIONS
The Hydro-static test tube side of the heat exchanger at 205 bar for a minimum of 60 minutes, as per ITP section 6.5
was noted as acceptable to the client requirements.
The ITP section 6.7 states that another Hydro-static test, shell side of the heat exchanger at 29 bar with shell cover
fitted is also to be completed, but this heat exchanger design does not have a shell cover. Poole process stated that

at 29 bar with shell cover fitted was not completed, as there is no shell cover to fit
No documentation was available to review [ except the hydro test certificate] as the QC engineer has since left the
company but it was stated by Poole process that the MDB will be ready to review by 21.09.2012
The test certificate was produced during the visit and is attached to the report
The heat exchanger is to be sent for painting on Monday 17.09.2012 and will be completed for final inspection for
21.09.2012 as per Poole Process Limited.
