Hai, How do you do? It’s nice to teach you in

this new academic year. I am Ms. Riana, I am
your English teacher for Bahasa dan Sastra
Inggris. I hope we can do good cooperation in
learning process. Today, in our first meeting,
we are going to study about something that
perhaps you usually see in our daily life, it’s
about ‘filling form’ / ‘mengisi formulir’.
Then let’s start our discussion.

1. What is Form?
A form is a 2 with spaces (also named fields or
placeholders) in which to write or select, for a
series of documents with similar contents.
The documents usually have the printed parts in
common. Forms, when completed, may be a
statement, a request, an order, etc. Forms may
be filled out in duplicate (or triplicate, meaning
three times) when the information gathered on
the form needs to be distributed to several
departments.
So we can conclude that, form means ‘formulir’
in Bahasa. Form is a printed document that needs
to be completed by statement, request, order, and
etc ( formulir adalah dokumen yang dicetak yang
perlu diisi/dilengkapi dengan pernyataan,
permintaan, perintah, dan lain-lain).

2. Types of Form
I think in your daily life, you have ever seen
many types of form.
