AFFIDAVIT OF COMPLIANCE

I, ELENA P. CONDE, of legal age, Filipino, widow, and a

resident of P-1B, Poblacion, Barobo, Surigao del Sur, after
having been duly sworn to in accordance with law, hereby depose
and say:

1. That I am the owner/proprietor of the H2G WATER SUPPLY

(formerly 7H Water Supply), a water supply business
situated at Poblacion, Barobo, Surigao del Sur, under
Water Permit Nos. 020553, 020554, 020555 & 02556 ;

2. That on June 24, 2019, the undersigned received the

Order of the Honorable Board dated May 21, 2019 ;

3. That pursuant to the said Order, the undersigned is

required to settled the unpaid annual water charges and
install water measuring devises/production meters and
submit record of monthly water withdrawals in the
prescribed form ;

4. That as to the annual water charges, the undersigned is

initially paying a partial amount of P________ and as to
the balance, hereby attached a promissory note to support
the same ;

5. That with regards to the installation of water measuring

devices/production meters, as well as its submission of
monthly record of water withdrawals, the undersigned is
confident that the same will be complied within this
month ;

6. That I am executing this affidavit to attest to the

truthfulness of the foregoing statements and as
compliance with the National Water Resources Board
requirement.

IN WITNESS WHEREOF, I have hereunto set my hands this 8 th
