KARU: "Assalamualaikum Wr.Wb... before we do Operand, let us give thanks for the presence

PA Pagi: " Assalamualaikum Wr.Wb,thank you for opportunity given to me to explain the
current condition of patients, the number of patients currently is 3 people.

1. patient number 1,identity name Mrs.D, age 25 years old, minimal dependency level care
Medical diagnosis of femoral fractur.The general stase of the patient is good.Last TTV at
01.00 p.m tens 130/80 mmHg, Temperature 37'c, pulse 86x, Respiration 20x, GCS 15.
Patient complans of pain in a broken leg. Nursing Problems found are discomfort, pain.

Implentation that has been done

1) Assess the level of pain with moderate pain

2) Maintaining bed rest, a quite environment little lighting

3) Minimize environmental disturbances and stimuli

4) Limiting activity

5) Give Analgesia and sedation medication according to order

Interventions that have not yet been implemented

1) Give pleasant actions according to indications such as ice packs comfortable position,
relaxtion technique, guidance of imagination

2) Avoid constipation

Evaluate (SOAP)

S: The patient said headache was reduced

O: The patient seems relaxed

A: the problem of pain is partially resolved

P: continue intervention
2. Patient number 2, Identity Name Mr.Q age 45 years old. Minimal Dependency level of
care.Medical diagnosis of fibular fracture the general stase of the patient is good.Last TTV
01.00 p.m tens 120/90 mmHg, Temperature 36'c, Pulse 90x, Respiration 20x, GCS 15.
Patient complains of pain in broken leg. Nursing problem found are discomfort pain.

3. POST KONFRENS

KARU:"thank you for your cooperation from all parties who have worked well. Thus, we accept
this, hopefully what we have done today will provide many benefits to us all and we will be
given fluency in carrying out our repective duites. Excitement for the picket in afternoon and be
careful on the way safely to destination for the morning service.Let's end this consideration by
