REHABILITATION OF DIVERSION DAM

This is to CERTIFY that I, JOSEPH E. DE LARA, Municipal Mayor and

representing the Peoples of Solsona hereby ACCEPT from RCD GENERAL
CONTRACTOR AND CONSTRUCTION SUPPLY the above-mentioned
project.

This is to FURTHER CERTIFY that the above-mentioned project has been

93% COMPLETED as per STATEMENT OF WORK ACCOMPLISHMENT
dated April 2, 2018 duly signed by former Municipal Engineer Cesar O. Rumbaoa,
and approved payment by Former Municipal Mayor Alexander A. Calucag (see
attached).

That the above-mentioned project is only 93% since the only available fund
as certified by the Municipal Accountant dated June 30, 2018 amounts to overall
TOTAL of NINE MILLION EIGHT HUNDRED SIXTY FIVE THOUSAND
PESOS comprising of EIGHT MILLION EIGHT HUNDRED SIXTY FIVE
THOUSAND PESOS (PHP 8,865,000.00) and ONE MILLION PESOS (PHP
1,000,000.00) representing the share from Department of Agriculture and
Municipal Government of Solsona, respectively, and there were NO
ADDITIONAL FUND credited in favor of the Municipal Government of Solsona.

This CERTIFICATION does not in any way relieve the contractor of any
obligation and responsibilities in undertaking the works of any structural defects or
failure that may occur in any section of the project during the warranty period
starting from the date of issuance of this certificate in accordance with Section
62.2.3 in relation to Section 62.2.3.2 of the Revised Implementing Rules and
Regulation of the Government Procurement Law a.k.a RA 9184.

This is to CERTIFY that Mx. CHARYMELLE O. FORONDA, Municipal Budget

Officer, is designated as Bids and Awards Committee Chairperson and Municipal Incident
Command System as Logistic Section Chief pursuant to Executive Order No. 08 s 2021.

The Philippines was declared by the President under state of calamity due to CoViD-19
and African Swine Fever. Consequently, MICS was activated.

The duties and functions of the logistic section is to provide resources and all other
services needed to support facilities, transportation, communications, supplies, equipment
maintenance, fuel, food services and medical services.

This is to further certify that the frequent contact to contract tracers and quarantiners to
provide logistic services gravely contributed on the exposure of Mx. Foronda.

That she become symptomatic and was advised to undergo test on August 4, 2021 and
yielded positive result.

That she underwent mandatory quarantine and was constantly monitored by Barangay
Health Emergency Response Team and the Rural health Unit.
