JOB DESCRIPTION - AGING

I. POSITION TITLE: Homecare Coordinator

II. PROGRAM COMPONENT: Senior Citizens Center

SUPERVISOR: Director

III. JOB SUMMARY: Provide direct personal care and support services to
homecare clients; records services provided to clients and assist with
scheduling services and inputting Homecare data.

IV. DUTIES AND RESPONSIBILITIES:

1. Directly responsible to Senior Center Director to coordinate plans with


BGADD for maximum delivery of services.
2. Steps in as PRN Homecare Aide when an aide is off on vacation etc.
Responsible for the following services: Homemaker, chore, personal care,
shopping, meal preparation and delivery when necessary, escort and
supportive services.
3. Will be organized to provide efficient and effective services at all times.
4. Will provide transportation in personal car or Agency vehicle when needed.
5. Will report other services in the home to avoid duplication and gaps in
service delivery.
6. Responsible for maintaining accurate documentation of the services
provided and inputting data into SAMS, the state database.
7. Will communicate with Homecare Case Manager and keep detailed
documentation.
7. Must maintain and submit all forms required by the Homecare policies and
procedures (10 phone calls and 5 home visits a quarter).
8. Will maintain mileage reimbursement records to coincide with daily activity
report.
9. This job description may not include all non-essential duties of the position.

IV. POSITIONS SUPERVISED: None

V. QUALIFICATIONS: Experience in working with the elderly preferred. Valid


Kentucky driver's license. Ability to get along with others.

VI. PHYSICAL QUALIFICATIONS: Physical demands are representative of those that


must be met by an employee to successfully perform the essential functions of this
job. Employee must be physically able to assist with transfers and/or lifting of clients
up to 100 pounds. Job requires sitting, standing, grasping, speaking, climbing,
pushing/pulling, carrying, reaching, stooping, and crouching. Clarity of vision and
the ability to smell is important. Ability to comprehend simple instructions and ability
to apply common sense understanding to carry out instructions furnished in written,
oral or diagram form. Ability to work effectively in one on one and/or small group
situations.
