Standaard Ponsetibehandeling:
The system of Diméglio is derived from a detailed scoring system based on the
measurement of four parameters:
1. equinus in the sagittal plane
2. varus deviation in the frontal plane
3. ‘derotation’ around the talus of the calcaneoforefoot block
4. adduction of the forefoot on the hindfoot in the horizontal plane
From the score, which has a maximum of 20 points, the deformity can be graded as benign
(<5), moderate (5-10), severe (10-15) or very severe (15-20).

The six signs are separated into three related to the hindfoot (severity of the posterior
crease, emptiness of the heel and rigidity of the equinus), and three related to the midfoot
(curvature of the lateral border of the foot, severity of the medial crease and position of the
lateral part of the head of the talus).
Thus, each foot can receive a hindfoot score between 0 and 3, a midfoot score between 0
and 3 and a total score between 0 and 6.
Gipsredressie
1. Je pakt de voet met een hand van bovenaf vast waarbij het belangrijkste is dat je
duim druk geeft op het laterale deel van de kop van de talus (bolling ca. 1 cm voor de
laterale malleolus) dit is het laterale deel van de kop van de talus. Het doel bij je
eerste gips is om de cavus eruit te halen. Equinus laten bestaan.
2. Nu brengen we de adductie in neutrale positie en halen we een deel van de
suppinatie eruit zodanig dat de cavus verder geredresseerd wordt. Het gips moet
onder de tenen iets verder doorlopen en de bovenkant van de tenen moet
vrijgemaakt worden. Een controlepunt van het gips is dat je vijf tenen moet zien, bij
een holle voet zie je de grote teen niet of niet goed.
3. Nu de voet zodanig deroteren dat de resterende supinatie automatisch opgeheven
wordt.
4. 40 graden exorotatie/abductie geven
5. Dit is het gips na achillespeestenotomie. Dit gaat gebeuren als de voorgaande
redressies volgens wens zijn verlopen en de voet voldoende is geredresseerd. Dat
wil zeggen de calcaneus in valgus, de taluskop bedekt en 50 graden abductie. Bij erg
stugge klomvoeten zijn er wel eens meer weken redressiegipsen nodig. Na de
achillespeestenotomie wordt de voet ingegipst in 15 graden dorsiflexie en 70 graden
exorotatie/abductie voor in totaal 3 weken.
De ouders wordt hierna geïnstrueerd hoe ze de Ponseti schoentejs en bar moeten aandoen.
Er moet goed gelet worden op voldoende laagstand van het hieltje.
Differentiaal diagnosen
Metatarsus adductus
- voorvoet in adductie, achtervoet neutraal
- als voet tot abductie te brengen is volgt spontane correctie
- als spontane correctie uitblijft, spreekt met van metatarsus
valgus (skew foot, komma voet) en moet gipsredressie
volgen
Operatieve behandeling:
Bij recidivering zijn vaak additionele ingrepen nodig, deze kunnen afhankelijk van de leeftijd
en stugheid bestaan uit weke-delen en/of ossale behandeling. Neglected club feet =
klompvoeten met eerste behandeling na zesde levensmaand.
Operatieverslag Turco:
Mediale incisie vanaf metatarsale 1, 1 cm onder de mediale malleolus door en aan de
mediale zijde van de achillespees naar proximaal voortzetten. Klieven subcutis. Opzoeken
van respectievelijk de volgende structuren: de posticuspees (deze wordt Z-vormig verlengd),
de pees van de flexor digitorum longus (vervolgen tot de knoopvan Henry waar deze kruist
met de flexor hallucis longus, de vaat-zenuwstreng (teugelen met vesselloop en
mobiliseren), de flexor hallucis longus en de achillespees. De AP wordt Z-vormig verlengd,
mediaal los van de calcaneus. Klieven achterste kapsel bovenste spronggewricht, indien
nodig openen kapsel talo-calcaneaire gewricht. Klieven achterste kapsel onderste
spronggewricht en lig. calcaneo-
fibulare. De posticuspees naar distaalvolgen tot in het talo-naviculaire gewricht. Dorsale en
plantaire kapsulotomie. Het naviculare wordt rondom goed los gemaakt, dusdanig dat nu het
naviculare kan worden gemanipuleerd en gedraaid rondom talus. Van achteraf worden de
calcaneo-talaire verbindingen en Y-ligament losgemaakt. Repositie naviculare t.o.v. de talus.
(Indien nodig met K-draden naviculare-talus en calcaneus-talus-tibia fixeren.) Adapteren
posticus en achillespees vicryl 2-0. Afnemen bloedleegte. Sluiten subcutis vicryl 3-0 en huid
met vicryl rapide 4-0.
Nabehandeling:
Bovenbeensgips voor 8 weken, na 2 weken gipswissel met wondinspectie
Complicaties:
Drukplekken van het gips, neurovasculaire schade, AVN van de talus, recidief klompvoet,
overcorrectie in planovalgus, ondercorrectie met restsupinatie bij lopen.
Bij een (dynamische) rest-deformiteit met een oversupinatie (verminderde actieve eversie)
van de voorvoet en klachten van de overbelaste laterale voetrand.

Paton et al. Is congenital talipes equinovarus a risk factor for pathological dysplasia of the
hip? : a 21-year prospective, longitudinal observational study. - 2014
Abstract
There is controversy whether congenital foot abnormalities are true risk factors for
pathological dysplasia of the hip. Previous United Kingdom screening guidelines considered
congenital talipes equinovarus (CTEV) to be a risk factor for hip dysplasia, but present
guidelines do not. We assessed the potential relationship between pathological dysplasia of
the hip and fixed idiopathic CTEV. We present a single-centre 21-year prospective
longitudinal observational study. All fixed idiopathic CTEV cases were classified (Harrold and
Walker Types 1 to 3) and the hips clinically and sonographically assessed. Sonographic Graf
Type III, IV and radiological irreducible hip dislocation were considered to be pathological hip
dysplasia. Over 21 years there were 139 children with 199 cases of fixed idiopathic CTEV
feet. Sonographically, there were 259 normal hips, 18 Graf Type II hips, 1 Graf Type III hip
and 0 Graf Type IV hip. There were no cases of radiological or sonographic irreducible hip
dislocation. Fixed idiopathic CTEV should not be considered as a significant risk factor for
pathological hip dysplasia. This conclusion is in keeping with the current newborn and infant
physical examination guidelines in which the only risk factors routinely screened are family
history and breech presentation. Our findings suggest CTEV should not be considered a
significant risk factor in pathological dysplasia of the hip.
Maripuri et al. Ponseti casting for club foot - above- or below-knee?: A prospective
randomised clinical trial. - 2013
Abstract
We undertook a randomised clinical trial to compare treatment times and failure rates
between above- and below-knee Ponseti casting groups. Eligible children with idiopathic
clubfoot, treated using the Ponseti method, were randomised to either below- or above-knee
plaster of Paris casting. Outcome measures were total treatment time and the occurrence of
failure, defined as two slippages or a treatment time above eight weeks. A total of 26
children (33 feet) were entered into the trial. The above-knee group comprised 17 feet in 13
children (ten boys and three girls, median age 13 days (1 to 40)) and the below-knee group
comprised 16 feet in 13 children (ten boys and three girls, median age 13 days (5 to 20)).
Because of six failures (37.5%) in the below-knee group, the trial was stopped early for
ethical reasons. The rate of failure was significantly higher in the below-knee group (p =
0.039). The median treatment times of six weeks in the below-knee and four weeks in the
above-knee group differed significantly (p = 0.01). This study demonstrates that the use of a
below-knee plaster of Paris cast in conjunction with the Ponseti technique leads to
unacceptably high failure rates and significantly longer treatment times. Therefore, this
technique is not recommended.
Gray et al. Is tibialis anterior tendon transfer effective for recurrent clubfoot? - 2014
Abstract
BACKGROUND:
Tibialis anterior tendon transfer surgery forms a part of Ponseti management for children
with congenital talipes equinovarus who, after initial correction, present with residual
dynamic supination. Although retrospective studies support good outcomes, prospective
longitudinal studies in this population are lacking.
QUESTIONS/PURPOSES:
We assessed strength, plantar loading, ROM, foot alignment, function, satisfaction, and
quality of life in patients with clubfoot that recurred after Ponseti casting who met indications
for tibialis anterior tendon transfer surgery, and compared them with a group of patients with
clubfoot treated with casting but whose deformity did not recur (therefore who were not
indicated for tibialis anterior tendon transfer surgery).
METHODS:
Twenty children with idiopathic congenital talipes equinovarus indicated for tibialis anterior
tendon transfer surgery were recruited. Assessment at baseline (before surgery), and 3, 6,
and 12 months (after surgery) included strength (hand-held dynamometry), plantar loading
(capacitance transducer matrix platform), ROM (Dimeglio scale), foot alignment (Foot
Posture Index(©)), function and satisfaction (disease-specific instrument for clubfoot), and
quality of life (Infant Toddler Quality of Life Questionnaire™). Outcomes were compared with
those of 12 age-matched children with congenital talipes equinovarus not indicated for
tibialis anterior tendon transfer surgery. Followup was 100% in the control group and 95%
(19 of 20) in the tibialis anterior transfer group.
RESULTS:
At baseline, the tibialis anterior tendon transfer group had a significantly worse eversion-to-
inversion strength ratio, plantar loading, ROM, foot alignment, and function and satisfaction.
At 3 months after surgery, eversion-to-inversion strength, plantar loading, and function and
satisfaction were no longer different between groups. Improvements were maintained at 12
months after surgery (eversion-to-inversion strength mean difference, 8% body weight; 95%
CI, -26% to 11%; p = 0.412; plantar loading, p > 0.251; function and satisfaction, p = 0.076).
ROM remained less and foot alignment more supinated in the tibialis anterior tendon transfer
group between baseline and followup (p < 0.001, p < 0.001).
CONCLUSIONS:
Tibialis anterior tendon transfer surgery was an effective procedure, which at 12-month
followup restored the balance of eversion-to-inversion strength and resulted in plantar
loading and function and satisfaction outcomes similar to those of age-matched children with
congenital talipes equinovarus who after Ponseti casting were not indicated for tibialis
anterior tendon transfer.
