Dear Sir/ Madam,

Novel Corona Virus Disease (COVID-19) – Arrangement of Isolation Rooms with


M/s Apollo Hospitals

Please refer to our letter CO. HRMD. No.16665/18.01.000/2019-20 dated June 18,
2020 and emails dated September 30 & December 24, 2020 on the captioned subject .

2. In this connection, it is informed that Bank has once again made tie-up
arrangements with M/s Apollo Hospitals for quarantine requirements due to COVID-
19, as they have resumed the facility of providing rooms in hotels in select cities (viz.
Mumbai, Delhi, Chennai, Hyderabad, Indore and Bengaluru) for the purpose.

3. The following are the features of the current arrangement:

i. An employee and his/her dependent family members, who is COVID-19 positive


or is having minor symptoms without confirmed infection but requiring isolation
can avail the facility. Staff of IT vendors/ service providers working on critical
operations of the Bank are also permitted to avail this facility, with prior approval
of In-Charge of the RO/ COD.
ii. The following are the city-wise applicable room rates, under the current
arrangement:

Mumbai Rs.4,000/-
Delhi Rs.3,500/-
Chennai Rs.4,500/-
Hyderabad Rs.3,500/-
Indore Rs.4,000/-
Bengaluru Rs.3,500/-

iii. The package at hotel includes three meals along with`vitals monitoring and
doctor consultation, virtual as well as physical, on a daily basis.

iv. Reservation has to be made by calling at 1860-500-0202 or by sending email


to projectstay-i@apollolifeline.com and drtarun_k@apollohospitals.com with
details such as name; contact number; office name; age; location; reason for
quarantine; booking for (self/family); chronic illness, if any. Upon confirmation,
one can directly go to the hotel for checking-in, however, information shall be
provided to concerned Office/COD about the same.

v. In the event of hospitalization requirement, M/s Apollo Hospitals will facilitate


the same, however, the onus to find a bed in hospital and related transportation
will be on the concerned person.

vi. Employee/ eligible family members/ vendor staff shall have to make payment
on their own, for which reimbursement may be taken subsequently, under
Bank’s Medical Facility Scheme.

4. As this facility is operational in select cities, Offices in other centres, may explore
similar arrangements in their area for tie-up, within the ceiling of Rs.4,000/- per night.

5. This arrangement shall be available till June 30, 2021.
