 An Operator must completely wear the Personal Protective Equipment (PPE’s) before
the start of the operation.
 Ensure that the equipment/s are fully functional and operational before the conduct of
the activity.
 Always maintain 3-point contact. (2hands and 1foot or 2feet and 1 hand).

 To get off the machine, make sure that the machine is parked, and further movement
prevented. Shut off the machine if necessary.
 Falling debris
 All workers must have a complete PPE'S: Hard hat, Reflectorized vest, safety shoes
and gloves, Safety eyeglasses.
 Installation of Reflectorized and Visible Warning Signages must be present.
 There should be an active spotter/ Helper for the Equipments and all falling debris
present in the area.
 Proper Installment of bollards and caution tape.
 There should be a traffic head/ flagman present during the activity, it should be
attentive to manage the traffic.

2. STRUCTURAL EXCAVATION

 CONSTRUCTION METHODOLOGY: Backhoe excavate at least 4m wide with 1m workspace for the
workers access to move freely at the excavated area, height of the excavation may varies.

 HAZARDS :

1. Falls into trench or excavation.


2. Tripping over equipment
3. Exposure to underground services or overhead electrical cables.
4. Incidents involving vehicles and other mobile equipment.

 PREVENTION:
1. Conduct tool box meeting focuses on safety topics on specific job, such as workplace
hazards and safe work practices & all workers must wear PPE’s.
2. Install regulatory signs, warning signs & bollards 4.5mts apart with reflectorized tape
interconnected by caution tape.
3. Spotter/helper assists the equipments movement.
4. Flagman must be visible on work site for safety traffic.

3. EMBANKMENT FROM STRUCTURAL EXCAVATION

 CONSTRUCTION METHODOLOGY: Embankment materials from riverbanks use backfill to the portion
of project that is needed for embankment.

 HAZARDS :

1. Incidents involving vehicles and other mobile equipment on site.


2. Resources or property may be covered by the excavated soil.

 PREVENTION:
1. Conduct tool box meeting focuses on safety topics on specific job, such as workplace
hazards and safe work practices & all workers must wear PPE’s.
2. Operator must be trained on proper handling of equipment.
3. Install regulatory signs, warning signs with reflectorized tape interconnected by caution
tape.
4. Flagman visible on work site for safety traffic.

 Each gabion mesh will be unfolded on a hard, flat surface. All kinks will be stretched and stamped out.
It will be made sure that all creases are in the correct positions for forming the box, one at the edge of
each end panel and each diaphragm.

 The side and end panels will be folded into the upright position to form a rectangular box. The top
corners of the box will be joined together with the thick selvedge wire which is sticking out from the
corners of each panel. This will make sure that the tops of all four sides of the box are level.

 Wiring up will begin by securing the binding wire at the top corners of the panels to be joined, by
looping it through and twisting it together. Then the wire will be laced around the selvedges with single
loops and double loops in turn at 100 mm intervals. Finally, the wire will be fastened securely at the
bottom corner, and the loose end will be poked inside the gabion. Next, the diaphragms will be lifted
into the vertical position and will be wired up to the side panels in exactly the same way.

 The lids will be tightly stretched over the filling and will be wired down. The corners will be temporarily
secured first, to ensure that there is enough mesh to cover the whole area. Some filling may have to be
removed from the top of the gabion to prevent the lid from being over-stretched.

GENERAL NOTES:

 Project Engineer must be present during critical activities at all time.


 Housekeeping must be perfume daily and garbage disposal area must be established.
 All operators or drivers must be experienced and skilled enough to perform critical activities.
 Tools and light equipment must be turned over to warehouse and heavy equipment parking area
must also be established.
 All workers must wear the required PPE’s for that activity (ex. Safety vest, hard hat, facemask,
gloves, safety harness, welding mask, welding gloves, safety shoes).
 COVID PROTOCOLS must be strictly followed at all times.
 Toolbox meeting must be conducted before any activities will commence.

These steps involve going through everything in a space, deciding what's necessary and what isn't, putting things in
order, cleaning, and setting up procedures for performing these tasks on a regular basis.

COVID – 19 PROTOCOL SAFETY

S:  SCREENING of all Assessors, Examinees, and staff in order to ensure safety for all.
 Complete a telephone screen via questionnaire prior to attending.
 Everyone who enters an Assess Med location is Nurse screened on a secondary basis prior to entering the
location via questionnaire and touch less temperature screening.
 Everyone is required to sanitize their hands, put on PPE including masks, gloves and shoe covers prior to entering
the facility.
A:  ADJUSTING our Workspace to maintain social distancing
 To accommodate physical spacing operating days and/or hours will be expanded.
 Appointment times are staggered to enable social distancing, screening and time for cleaning assessment and
common areas/equipment.
 Waiting and assessment areas have been rearranged to maintain 2m physical distancing with signage and floor
decals as reminders.
F: Focused efforts on enhanced cleaning and disinfecting our offices
 Plexiglas screen guards have been installed at all locations in both reception and interview rooms.
 Staff are responsible for cleaning the equipment and high touch surfaces in between each appointment in all areas
of the locations.
 Work stations and all equipment are sanitized before and after use or as needed throughout the work day,
including high touch surfaces such as desks, phone, computer, printer, pens, door handles etc.

E: EFFECTIVE usage of Personal Protective Equipment

 Everyone is required to sanitize their hands, put on PPE including masks, gloves and shoe covers prior to entering
a facility.
 Education provided on the proper use of PPE including donning and doffing procedures.
 Clinicians are required to wear and change gloves between each assessment.
 Assessors are provided with face shields as required for physical exams.
