in the Conduct of Accounting, Monitoring and Periodic Audit of Pensioners for
the Protection of the PNP Pension Fund”; and
b. PNP Memorandum Circular No___ entitled “Guidelines and Procedures of the
“Pensioner Ko, Sagot Ko” Project.

2. Respectfully submitted are the following:

a. Recapitulation/Matrix of the Individual Initial Contact, Wellness Check and

Visitation Certifications of the deployed Active Personnel-Buddies of this
Station {Office or Unit}; and

b. The original copy of the above Certifications, Update Forms, Affidavit of

Undertakings, Pictures, and documents from the PSA or the Local Civil
Registrar.
