COMES NOW, the complainant, and unto this Honorable Office,
respectfully avers that:

1. The office received the Notice of Decision on August 15, 2019

notifying the office that a Decision was already rendered on June
28, 2019, to wit:

“WHEREFORE ABOVE PREMISES CONSIDERED, we find that

complainant CIRILO B. YAUN to have been illegally dismissed sans just
or authorized cause and with non-observance of due process.

We shall hold respondent TACLOBAN GLEEN MARKETING INC. liable

for the payment of the total judgment award due to the complainant, to
wit:

a. Backwages – P139,732.00
b. Separation Pay – P 110,396.00
c. 13th month Pay- P 2,759.00
d. 10% of the total judgment award as attorney’s fees.

 2. The respondent TACLOBAN GLEEN MARKETING INC., changed its
address from Door IV Friendship Building, Subangdaku, Mandaue
City to SAN ANTONIO VILLAGE, LAHUG APAS, CEBU CITY.

PRAYER
WHEREFORE, in view of the foregoing, it is most respectfully
prayed unto this Honorable Office to make record of the change address
of the respondent.

Other reliefs just or equitable under the premises.
