PED MODULE 1 ACT. 1

As a trainee, I interviewed two (2) individuals, one of my clients are my relative


(first-person) and the other is not connected to the bloodline, who have suffered from
some chronic or debilitating condition that needs maintenance drugs and limitations in
the performance of their daily living activities.

I interviewed my aunt who suffers from some chronic or crippling condition that
requires maintenance drugs and limitations in the performance of his daily living
activities last November 18. I asked him a few questions, and she answered all of them,
and all of those were his answers:

NARRATIVE REPORT
I’m Felipe 55 years old currently living at Brgy. Cogon Pardo, Cebu City. I had a
medical condition and that is Bronchial Asthma. I have lots of desire that I wanted to
achieved in life and I know that those desires will help my medical condition. I draw a
road map to serve as my guidance for achieving the future outcome I want. I draw the
road map by picturing out my life in the future and go into details about the ultimate
outcome I’d like to achieve and I make sure to include the reason why I want to go on
this journey. Then, I plan the outcomes for me to have a direction in life and to be
healthy and physically fit. And we need to prepare ourselves for failure and don’t get
disappointed upon failing because you can enjoy your growth and take away valuable
lessons even if you fail. Next, I plan the actions that we need to take and then do it in
order to achieve the desired outcome and always think what would be the possible
challenges or obstacles you encounter. If you encounter any obstacles just face it with
courage because your goal is to be healthy. The factors that lead to the fruition of my
life’s success is to be a self-confident person, building your confidence and working out
what will really give meaning and fulfillment to your life. Determinants of success that
make me happy are my strong relationship to my family on how we support each other.
And my families are the big contributors of my success. My family is my source of
everything they are my strength to do everything I can.

And this road map I make it really helps me to have a healthy lifestyle because
this serves as my guidance on what should I do and I’m looking forward to the future of
what is the outcome of my actions. I thought of attaining what I had drawn before in my
road map for me to be fit and healthy. Since, I’m a person who suffered asthma for how
many years already and I’m thankful that I experience it sometimes because I also do
have a check-up if it triggers. I have a relative side from my mom who also had this

medical condition. As far as I know the disease that are common in my family are
asthma and high blood pressure.

I’m not indulging myself in the use of illicit drugs, excessive consumption of
alcohol, chain smoking but I sometimes skip adequate sleep over work and spending
too much of worrying. But I managed to take care of myself by avoiding it because it’s
hard to be sick I have a family to sustain their needs. I plan to have physical activity
which is to exercise regularly but since I have work I can’t have exercise daily because
of being busy. I start exercising early but not in a regular way.

My doctor advised me for my diet is to eat Vitamin D-rich foods, such as milk and
eggs and avoid those forbidden food to cause of trigger of asthma and, to medications I
have a medical prescription for my asthma and I do some physical activity such as
swimming. If I were to look back, the lifestyle practices I would have changed to prevent
the onset of my present medical condition is that I should have been took care of myself
more and doing exercise regularly to be healthy and physically fit because this is for me
it will really help to achieve the outcome that I need for myself.

Last November 18, 2020 I interviewed my neighbor who is suffering from any chronic or
debilitating disease that requires maintenance medications and limitations in the
performance of his activities of daily living. I asked him a couple of questions and she
answered it all and these are all his answers:

NARRATIVE REPORT
I’m H.Q years old currently live at Pardo, Cebu CIty. I had a medical condition
and that is Asthma. I have goals in life and that will help me for my medical condition
and for me to have a healthier life. I draw a road map or wrote plans for the future
outcome that I want to achieve which is to be healthy. I draw the road map by focusing
on my goals in life, then plan for what outcomes you like to reach and plan for what
proper actions to do for it to be achieve then after do the actions to reach it. The
essential factor that leads to the fruition of my life’s success is gratitude. Gratitude
works on many levels and is not as simple as giving thanks for that which you have. It is
also important to acknowledge why you are grateful for the things you choose to list.
The determinant of my success that makes me happy is seeing my family happy. And
my families are the contributors of my success they are my strength to have a peaceful
life and to achieve my goals in life for myself and for them.”

This road map or my plans that I make it really helps me to have a healthy
lifestyle because this would serves me as my guidance on what should I do or what
proper actions to take for me to reach the goals that will help myself from the disease
I’m experiencing. I want to succeed or achieve the plans that I made before for me to
have healthier life and be physically and mentally fit. The medical condition I have is
asthma and it only occurs sometimes I’m still thankful for it. I have a relative also that
experience this. The disease that is common in my family is asthma.

worrying. But now I already manage to take care of myself I eat healthy foods and avoid
everything that triggers asthma. I do exercise but it’s not regularly since I have work
every day even though its weekends so I don’t have really time for exercise.

The doctor advised me to eat healthy food and avoid the forbidden food of the
person have asthma. I have medications also that were prescribed by my doctor. I also
do some exercise but not regularly. If I were to look back, the lifestyle practices I would
have changed is to care for myself more and have a regular check-up if I am really
healthy and most important have some exercise.

Success and Happiness

Each of us has different definition of success and happiness. Success is when


you fulfilled everything you set out to do in life and are happy with what you are doing.
And happiness is something which we can’t describe in words it can only be felt from
someone’s expression of a smile. So today I’m going to defines success and happiness
and what are the things made us to be happy and successful. Recognize also the
stumbling blocks, and changes.

Success is all about the feeling of satisfaction we get when we accomplish a task
or do something, it is the happiness we get just by remembering what we have achieved
and how we did it, success has to do with a lot of positive feelings about ourselves and
pride in what we have been able to do. Happiness represents how we feel it is
perceived to be the key to success. Happiness comes from personal accomplishment
and it is essential for our well-being. There are things that make us happy and to be
successful in life and it will be define through tangible and intangible things. The
tangible ways that make us to be happy and help us to succeed are our families and the
people around us who consider us as important person they will be our strength for us
to do everything we can. And the intangible things are how my family supports me and
understand to everything I want to achieve.

Recognize the stumbling blocks in the pursuit of their happiness and success.
We all have them or have experienced them at different points in our lives. They are
barriers that we either come up against, create in our minds, or allow others to pass on
to us and they stop our movement forward. The most ubiquitous blocks we have tend to
come from a number of emotional upsets; EXPECTATIONS we hold about how others
perceive us or how we wish to be perceived and sooner or later this creates a
dichotomy in our thinking that leads us down a path of self-delusion and not speaking
our truth. CONFUSION that mars our ability to make clear choices and this type of
stumbling block creates chaos and indecision in our lives. And there’s a quote from
Eleanor Roosevelt that help us or serve as our motivation “A stumbling block to the
pessimist is a stepping-stone to the optimist.”

The situation in the life of someone that needs to be changed is how the person
shows eagerness to achieve what they really want. We need to change it since we can
be a successful person through being happy with the life we have right now and with our
families. The changes would effect if the persons accept the fact that there are ways to
be successful in life.

The truth is that success does not result in happiness, but happiness can lead us
to success. Even though people assume that happiness is more important than

success, while other felt that they cannot experience happiness unless they are
successful. Upon achieving happiness and success we experience to have some
stumbling blocks but instead of facing it with fear use it as the motivational of having a
life you wanted to achieve.

Interpretation:

I can say that I’m currently physically active because of my work and I used my
free time to do something that engage physical work or activities and I do exercise to
make my body be more active and have lots of energy and for me to avoid being easily
felt tired. I even go to our farm every Sunday for me to have some breath of fresh air. I
intend to become more physically active in the next six months to be healthier since I’m
now turning 50 I want to do physical activities to avoid my body to getting weaker. I
currently engage myself in regular physical activity like walking and hiking. I never have
been regularly physically active for the past 6months because I was so busy with my
work. I have lots of things to do.

Conclusion:

Therefore I conclude that, while answering this activity I realized that I already
started doing things and have some exercises that would make me to have a good
outcome of my actions. And I want this to continue for having a healthier life and be
physically fit because it is really hard to be sick especially these days and also it is so
hard for me because I have a family that I need to sustain their needs.

Interpretation:

As of these days I am currently physically active but not to the point that I’m
doing it regularly since I have work, my work starts at 8 end at 8 and I work as a security
guard, I even work during Sunday so that’s why I can’t really have some exercises for
myself but instead of doing exercise I just eat healthy foods and avoid drinking alcohol. I
never have been regularly physically active for the past six months but I intend to
become more physically active in the next 6months.

Conclusion:

I conclude that because of having not enough time for myself I never realized
that I need to have some time for taking care of my body I know that I’m physically
active but still we need to continue this and do it regularly. Being physically active it
gives us to manage our weight better, have strong bones, muscles and joints and have
lower blood pressure these are the benefits of being physically active so in order for me
have these outcomes I need to be more engaged in doing physical activities for myself.
