Dear respondents,

The researcher would like to ask permission to conduct a survey that requires your full
cooperation and effort, as time convenience will be acknowledged, thus, handing the surveys
within the period of leisure. The study entitled “The Effectiveness of Modular Modality
among Senior High School (SHS) students of Pampanga High School on their Academic
Performances” shall need respondents to gather the required data for the study.

The researcher will ensure the confidentiality of each respondent and shall take necessary
measures for the privacy of each respondent, as this will only proceed to participants willing to
take the survey voluntarily. The demographics and profiles of participants shall be disclosed and
will not be displayed on the paper itself and to the public.

The researcher would like to conduct a simple survey that showcases open-ended
questions to gather the necessary data for the researcher’s qualitative study entitled “The
Effectiveness of Modular Modality among Senior High School (SHS) students of Pampanga
High School on their Academic Performances.”

The distribution of each survey shall be through online platforms to contact each participant. It
was ensured that any confusions were to be addressed, and administer each response responsibly.
The privacy and security of each respondent were acknowledged, thus, following a policy of
confidentiality that discloses their identity to the public.
