• If a printer machine or a print shop is available in your place, then you can print out this
test questionnaire. Otherwise, you can write your answers in a separate sheet of a long
bond paper. (Much better to print it out.)

• Avoid reproducing this TQ for a number of copies without my permission. Anyone who will
be caught doing so will be scored “0” zero in the said examination.
• Students who will receive this TQ must be paid for his/her “First Preliminary Examination” in
order for your scores to be recorded. Otherwise, the score will not be recorded unless the
student will pay the said First Preliminary Exam account payables.
• You are advised to compile this one together with your First Preliminary activities and must
be put in a brown envelope upon submission. In the envelope you should write your FULL
NAME, COURSE DESCRIPTION OF THE SUBJECT and CLASS TIME, AND THE NAME OF YOUR
TEACHER.
• Since I have two English 19 subjects this time, make sure to write the COURSE DESCRIPTION
of this subject for easy identification. Not following instructions will be deducted.
