PERFORMANCE TASK

INSTRUCTIONS: In this activity, come up with Risk Management Plan for your recreational game
implementation by filling in the following details.

Activity: Tapon sa Dilim

Planned Site of the Activity: Garage in Alamia Residence: 128-D Masuhud Drive, Talon-Talon,
Zamboanga City 7000
Risk Management Plan
Hazard
(Identify the specific source of hazard)
1. Human-induced risks, according to Van Westen (2013), are any variables that increase
the damage and pose a risk to humans, animals, and any earthy materials. With Tapon sa
Dilim as the recreational activity to be done, the participants are an essential part of the
game. A hazard from the participants is their (a) medical history, where the participant
Factors coming might have a history of asthma, anxiety, dizziness, and fatigue that could lead to harmful
from participants:
effects. Secondly, (b) the participants' lack of preparation and knowledge in terms of the
game, mechanics, weather conditions, and more could also bring adverse effects on the
participants themselves. Lastly, (c) unacceptable behavior of the participants before,
during, and after the activity.
2. Environmental hazards are any factors caused by natural and ecological events on Earth
that may possibly cause adverse effects on humans and other valuable things. Tapon sa
Dilim also needs an environment where players should play the game. The game will be
played in a covered garage, fitting two cars. One of the environmental hazards that may
cause harm is (a) the weather—maybe rain or hot temperature—that the day of the game
Environment
factors: might have. Extreme weather may be experienced by the participants, which may cause
severe problems for the health of the participants. Secondly, (b) the garage floor is dusty
and, sometimes, slippery. Lastly, (c) an environmental factor that could affect the game
is the presence of bees, wasps, and insects, in general. The neighborhood had a history
of bees, which is why bee stings are part of an environmental hazard.
Equipment: 3. Although human-induced and environmental hazards are essential hazards that need to
be emphasized, equipment hazards causing harm to the participants during the event
could also be detrimental to the participants and people around the location. Equipment
hazards are any resources that impact the activity. Specifically, the recreational activity
uses slippers and an empty can. One hazard from these equipment factors is (a) the
slippers. As the participants would be blindfolded, the slippers could be a hazard for the
other participants who are not yet playing the game. Additionally, (b) sharp equipment
—in this case, an empty can—could be a hazard to the participants too. Lastly, (c)
inappropriate clothing that could cause harm when participating in the activity is also an
equipment hazard.

Risk
(Identify a specific risk that the hazard my bring)
4. Based on the human-induced hazards mentioned above, these are the risks that the
participants may encounter. Firstly, (a) the participant with medical history could
experience an asthma attack and anxiety attack that could lead to the student fainting and
unable to breathe. In the worst-case scenario, the participant might need to go to the
Factors coming hospital through an ambulance. Secondly, (b) lack of preparation would lead to physical
from participants:
injuries and an unsuccessful activity—as the participant does not know how to play the
game, there may be injuries and arguments between the players. Lastly, (c) unacceptable
behavior could cause injuries and fights on the participants. If participants behave
insensitively, they might throw slippers on other participants, leading to minor injuries.
5. With the aforementioned environmental hazards, risks coming from these hazards could
cause harm to the participants. (a) Scorching weather could lead to heat strokes, fainting,
and dehydration, which could affect the activity. The participants could experience
severe injuries when they faint. On the other hand, extreme rain could cause tripping and
Environment
factors: sliding of the participants. Additionally, (b) dusty and slippery floors would cause slips,
trips, and falls, leading to bodily injuries and allergies from the dust that would cause
runny nose or even transmission of bacteria. Lastly, (c) bee and wasps sting could cause
allergic reaction, pain, swelling, nausea, and even trouble to breathe.
6. Given the equipment hazards mentioned, the participants and people in the event could
experience adverse effects, especially on their bodies. (a) The use of slippers could cause
injuries to the participant when the player throws the slipper accidentally on the other
participants. It could cause bodily injuries, pain, and bruises. Moreover, (b) sharp
equipment, like can, could lead to cuts, incisions, and bleeding of arms and other body
Equipment: parts that could be affected. The top edges of the can are sharp, so once the participant
holds the can on the opening, their hand could bleed because of it. Lastly, (c)
inappropriate clothing, like not using jogging pants and comfortable clothing, could lead
to head and bodily injuries. An example is a participant who uses too oversized pants,
which would lead to trips and falls when the player steps on the pants.

Risk Assessment
Factors coming 7. As observed from the hazard and risks of human factors, these can cause adverse effects,
like injuries and death, even simply by human factors. In the worst-case scenario, these
human hazards and risks could be even worse if environmental and equipment factors
were present. An example is a player with a history of asthma attacks who would faint

from participants: with a scorching temperature and improper clothing, like wearing sweaters. A simple
scenario like that would lead to adverse injuries and affect the whole game. As potential
risk factors are not mutually exclusive, organizers and planners should do a risk
reduction strategy to avoid these effects and have a successful Tapon sa Dilim activity.
8. With environmental risks being one of the most dangerous ones, these said hazards could
cause detrimental effects on the health of the participants of the activity. Similar to the
risk assessment of human-induced factors, environmental factors would lead to a worse
situation when combined with human and equipment factors. If the participant is on a
Environment dusty and slippery floor, they could slip and fall, considering that they also did not
factors:
prepare beforehand what to expect during the activity. With that, if the unprepared player
starts the game, they might hold the can on the sharp edges, which would lead to cuts and
bleeding. Undoubtedly, organizers will solve these aftermath events through proper risk
reduction strategies that the organizers and participants should do.
9. Equipment factors could also cause many injuries and other valuable things to the
participants. When combined with human-induced and environmental factors, these
equipment factors could cause adverse and depressing effects during the activity. If the

Equipment: blindfolded participant uses the slippers, they might throw them on the other participants.
Once faced with bee stings, the participant might resort to unacceptable behavior,
starting an argument and fighting between the participants. Indeed, that situation could
be avoided with proper risk management and a risk reduction plan.
Risk Reduction Strategy
(What should be done to reduce or manage the risk)
Factors coming 10. As mentioned, these hazards could cause severe effects on the participants. One way to
from participants: reduce and manage the risk is (a) the organizers have to ask for the participants' basic
profile and medical history to know if they have asthma, anxiety, and more. If the
participant has a bad medical history, the game masters should inform them not to attend.
If the participant persists, the leaders would be prepared to bring for the participant to
feel comfortable during the game. Additionally, health and medical details are the
responsibility of the organizers, which should be made clear when they enter the activity.
Secondly, (b) conducting tons and tons of meetings, preparations, and revisions on the
plan. By doing meetings and proper planning, the perils would basically not happen. As
conducting a pre-trip inspection of the site's location is considered planning, doing this
will cover the things to be prepared before going to the area. The organizers should
remove any valuable items to prepare the location. Lastly, (c) unacceptable behavior will
be isolated, preferably by a leader or organizer. The individual must be taken away from
the situation and calm down before being permitted to join back in. The organizers
should brief the players on what must be done before, during, and after the activity.
11. Mitigating the risk carried by the environmental factors is essential to every participant
and organizer in the event. Firstly, (a) to reduce the risk of weather conditions, extreme
temperatures could be risk-reduced by wearing good clothing suitable to the weather,
bringing personal water bottles or containers to avoid dehydration, and briefing on what
to expect with the weather. Furthermore, the organizers must assess weather conditions
Environment regarding health and safety to participants and cancel the activity, if necessary. (b) A
factors:
dusty and slippery floor could be mitigated by simply cleaning the place before the
event. Once the area is squeaky clean, the players will be able to play in the area. Lastly,
(c) to avoid the harm that bees and wasps bring, there must be a supply and provision of
insect repellent. The organizers should also ensure that the game area does not have any
ant nests, beehives, and other insects before commencement.
12. Considering all the hazards and risks that the equipment brings, the organizers should,
overall, quality-check all the equipment that will be used. (a) To avoid injuries from
slippers, the organizers should guide the players on where to throw the slipper, even if
the players are blindfolded. The best countermeasure that can be done is telling other
participants to stay back first and guiding the players that are currently in the game.
Additionally, (b) the sharp edges of the cans can be avoided by briefing the participants

Equipment: on how to handle the can—such as telling the participants to hold the can on the sides
and not on the opening of the can. The best measure that organizers and players can do
here is to wear gloves or anything that can protect their hands from getting cuts. Lastly,
(c) the organizers should also brief the participants on the most suitable attire for the
event. Participants need to be asked to wear proper clothing for activity prior to the event
in the material preparation process of the event. Sharp objects such as jewelry are to be
removed before participation.
