- LAST WILL AND TESTAMENT

- A last will and testament is a legal document that communicates a person's
final wishes pertaining to assets and dependents. A person's last will and
testament outline what to do with possessions, whether the deceased will
leave them to another person, a group or donate them to charity, and what
happens to other things that they are responsible for, such as custody of
dependents and management of accounts and financial interests. Some
states do allow for non-standard or unusual wills, such as a holographic will,
while others do not.

- HEALTH CARE POWER OF ATTORNEY

- A healthcare power of attorney (HCPA) is a legal document that allows an
individual to empower another person to make decisions about their medical
care. A healthcare power of attorney refers to both a legal document and a
specific person with legal authority.

- FINANCIAL POWER OF ATTORNEY

- A financial power of attorney (POA) is a legal document that grants a trusted
agent the authority to act on behalf of the principal agent in financial matters.
The former is also referred to as the attorney-in-fact while the principal-agent
is the person who grants the authority. This kind of POA is also referred to as
a general power of attorney.

- GUARDIANSHIP DOCUMENT
- Guardianship documents are state-specific legal documents used to
sign over the legal responsibilities of parenting or taking care of an
incompetent adult to another person. It's important to keep in mind that
guardianship forms aren't necessarily a substitution for filing a
guardianship case with the family or probate court in your area. When
completing guardianship forms, make sure that they state whether the
switch in guardianship is temporary or permanent.

- LIVING WILL
- A living will be a written, legal document that spells out medical
treatments you would and would not want to be used to keep you alive,
as well as your preferences for other medical decisions, such as pain
management or organ donation. In determining your wishes, think
about your values. Consider how important it is to you to be
independent and self-sufficient, and identify what circumstances might
make you feel like your life is not worth living

patient, their present status, and the reasons for their discharge. A discharge
narrative is important for keeping patients safe when they leave the hospital.
"Hospital discharge summaries serve as the primary papers transmitting a
