An Instructional Guide to complete the Helpdesk Form created by

DICT Mindanao Cluster 2 - Cybersecurity Bureau.


INTRODUCTION:
This Helpdesk Form was created by the Cybersecurity Bureau of the DICT
Mindanao Cluster 2 to efficiently respond to the PNPKI Users in Caraga Region and
Region X, as the email inquiries about PNPKI online registration and errors are getting
exponential.
Starting March 10, 2022, all concerns about PNPKI accounts of the users in the
abovementioned Regions shall be coursed to this form. Our Registration Authority
Officers (RAO) will also start directing to this form’s database to resolve those issues
instead of going through each email.

PROCEDURES:
STEP 1
Open your preferred web browser (Google Chrome, Mozilla Firefox, Microsoft Edge
or Internet Explorer) and copy this link: https://bit.ly/PNPKIHelpDeskForm
STEP 2
Fill out the form accurately starting with your preferred e-mail.

STEP 3
Continue filling out the form accordingly. Make sure you also enter the E-mail Address
you have initially used during your PNPKI registration. Otherwise, we may not be able to
track your account.

Note: For Security purposes, we may also not be able to provide any account-specific
information/link to your desired e-mail if it doesn’t match with the email registered on file.

Fields with Red asterisk (*) should NOT be left empty.


STEP 4
Organization/Agency field should be filled out completely. Instead of using DOH for
example, please enter ‘Department of Heath’
The next field is for DepED employees only (Admin/Teaching Staff) otherwise enter
‘N/A’

STEP 5
The next field is where you choose your concern about your PNPKI account.
STEP 6
Next fill out the deadline date of your concern if any. Leave blank if the inquiry is NOT
URGENT. Anyway, your ticket will still be on queue. Take note of the disclaimer.

STEP 7
The next field is also required. Make sure to specify the actions you have taken so far to
resolve your issue on your own. This includes the number of attempts you made to do
those actions and the last time you have used your PNPKI access prior to reporting your
concern.
This is where our RAO relies in identifying the action/s they also need to take on their
end.
STEP 8
Click the ‘Next’ button

STEP 9
You will be routed to this section ONLY if you have chosen ‘Request for Revocation’ in
the Concerns field.
Just Click the given link and download the form from the link. And then, click ‘Next’.
STEP 10
All other concerns will route you to this section. Click ‘Submit’ to finally file your ticket and
wait for the email notification of your next step/s to resolve your issue.

Note: If you haven’t received an email response from the PNKI Team within the given time frame,
you may need to check your email’s SPAM or JUNK folder. Otherwise, you may have used a
different email during your PNPKI registration
