AFFIDAVIT

I, MICHAEL D. O’HARE, of legal age, marriage status, citizenship

please verify, and a resident of _________________________, Misamis
Occidental, after having been duly sworn under oath, in accordance with
law, hereby voluntarily depose and say:

1. That I am the Reverend Founding Pastor of Oikos Christian

Fellowship of Oroquieta City, Inc., with principal address at Senator J.
Ozamis St., Lower Langcangan, Oroquieta City, Misamis Occidental;

2. That I am the registered solemnizing officer under the

aforementioned religious organization with a certificate of registration
confirming that I am hereunder authorize to solemnize marriage in the
Philippines, with Registry No. 2021-29HC335CGV-2023 issued on 13
November 2020 at Cagayan de Oro City, Philippines, hereto attached as
Annex “A”;

3. That I have solemnized the marriage between REYNALD

ANGELO CERVITO and RIALYN CORNEPILLO VERULA, all of which
are of legal age at the time of the celebration of their marriage, and without
any impediments under the Family Code of the Philippines, and the laws.
The Certificate of Marriage of Reynald Angelo Cervito and Rialyn
Cornepillo Verula is hereto attached as Annex “B”;

4. That the marriage was celebrated on a Monday, 26 September

2022 at four o’clock in the afternoon at Bakers den Resort in Libona,
Bukidnon, before at least two witnesses of legal, on which the contracting
parties, in their presence, have solemnly take each other as husband and
wife, hereto attached is the invitation card of the Wedding Celebration and
marked as Annex “C”;

5. That there were erroneous entries and/or clerical errors found

thereon aforementioned certificate of marriage of Reynald Angelo Cervito
and Rialyn Cornepillo Verula, which are the following:

a) That in No. 19 thereon as to the date of issuance of the marriage license,

the issued date entered was September 12, 2002 - 2022 instead of September
12, 2022 only;

b) That in same No. 19 thereon the Registry No. And Expiration date was
2021-29HC-335CGV-2023, instead of 2021-29HC335CGV-2023.

6. That I am executing this Affidavit to make the necessary the

necessary corrections of the erroneous entries in the certificate of marriage
Reynald Angelo Cervito and Rialyn Cornepillo Verula; and
 7. That to verify that the marriage of Reynald Angelo Cervito and
Rialyn Cornepillo Verula was celebrated on September 26, 2022 and in
whom I, with the authority conferred upon me under the law, have
solemnized in the presence of at least two (2) witness in accordance with the
Family Code of the Philippines and the laws governing marriage;
