Perceived Readiness Of Incoming College Students That Are Experiencing Pandemic

Research Instrument

Greetings!
Dear Grade 12 Students,
You are invited to participate in a research study about the Perceived Readiness of incoming
college students that are experiencing pandemic. This survey is being conducted to collect data
about your level of preparedness in this coming college year amidst of the pandemic. Your
participation in this survey is voluntary, and you are free to withdraw your participation from this
study at any time.
The procedure involves filling an online survey that will take approximately 15 minutes. There
are no risks associated with participating in this study. This online survey will ask for your
statistical data only and we do not collect identifying information such as your name, email
address or IP address. Rest assured that all of the recorded information and data will be kept with
confidentiality. You will not experience any direct benefits from participation. However, the
collected information in this survey will benefit our group (Grade 12 STEM-Einstein Group 7)
for our research paper.
The results of this survey will be presented at our online defense. If you agree to participate
please choose YES button at the bottom of the screen. Thank you.

PRIVACY CONSENT
In compliance with the Data Privacy Act (DPA) of 2012, and its Implementing Rules and
Regulations (IRR) effective since September 9, 2016, I allow the Grade 12- Einstein (Group 7)
to store and use my data for the purpose stated within the document. Upon filling out the form, I
give my full consent and authorization for the Grade 12- Einstein (Group 7) to:
1. Use my personal data for any research studies to be conducted by the Grade 12- Einstein
(Group 7) for 2021-2022
2. Access will be limited to the selected students personnel;
3. Improper use of the information in this form (online, offline, or printed) will be subjected to
appropriate actions under the Data Privacy Act of 2012.
I understand and am assured that necessary precautions will be taken by the authors to protect
my personal information.

2. I am ready to face struggles that comes with responsibilities in the next school year.
2.1 I am always bothered by the noise in our house.
2.2 I cannot focus on our lessons because of many distractions in our place.

2.4 I am willing to cope up with the new learning system even though I am in a new
environment.
2.5 I have my own gadget to use when in need.
2.6 I am just borrowing laptops and gadgets from my friends and family for me to join
the online classes.
2.7 We have a stable internet connection.
2.8 Mobile data is what I utilize for my online classes.
3. I am anxious and worried for the next school year.
3.1 Given today’s pandemic, our family is still great in terms of the financial aspect.
3.2 I am struggling with my mental health nowadays because I frequently talk and see my
friends and loved ones.
3.3 Different pain such as backpain and tiredness of eyes are what I’m feeling everyday
after a whole day of online classes and requirements.
3.4 I am really emotional towards everything because of stress and workload.
4. There are many barriers and obstacles that I must face for the upcoming school year.
4.1 I am really nervous about getting to know new people.
4.2 I am worried if I will be able to cope up with the new learning method especially in college
life.
4.3 I am thinking about the new adjustments that I would make just to not be left behind in terms
of my education.
