JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

KNOW ALL MEN BY THESE PRESENTS:

We, FERMIN T. DIAZ, Filipino, of legal age, with residential address at Canlalay,
Biñan City, Laguna and ALBERTO C. OLIVAREZ, Filipino, of legal, with residential address
at 151 Brgy. Tubigan, Biñan Laguna after having been sworn in accordance with law hereby
depose and state:

That we personally know ANGELITO BALIDOY DESIDERIO who died on January

2, 2023 Son of CLOVERIA SALLO BALIDOY;

That in the record of his Social Security System Membership (SSS) his mother’s name
appears as ZENAIDA B. DESIDERIO instead of CLOVERIA SALLO BALIDOY;

That the discrepancy in his mother’s name is mainly due to his husband OSCAR
CARAVANA DESIDERIO decided to change her name to ZENAIDA B. DESIDERIO at the
time they were registered;

That we are executing this Joint Affidavit of Two Disinterested Persons for the purpose
of affirming and confirming that ANGELITO B. DESIDERIO with Mothers name ZENAIDA
B. DESIDERIO and CLOVERIA SALLO BALIDOY are one and the same person.

IN WITNESS WHEREOF, we have hereunto affixed our signatures this 9 th day of

February, 2023 at Biñan City, Laguna, Philippines.
