SUBJECT: LETTER OF INTENT FOR FLEXIBLE WORK ARRANGEMENT

Dear Sir/Madam:

We are Coco Street Fashion Corp., an exporter of fashion jewelries mostly made from
coconut shells beads and other local materials, located in Talamban, Cebu City, with a
total of 3 workers.

Due to the coronavirus pandemic, no orders have been coming in as our customers are
also on quarantine and lockdown. This greatly affected us as we entirely depend on them
for orders. The Company is reducing the number of work days per week from 6 days
down to 4 days, effective this week March 27, 2020 until the COVID-19 situation is
improved, resolved or if there is a Government order that tell us otherwise. With this
decision, the company employees will be greatly affected by this slowdown and will need
DOLE’s financial assistance.

We are hereby submitting herewith our documentary requirements, Establishment Report

on COVID-19 and the list of affected workers.
