Re : REQUEST FOR PULL-OUT OF HEAVY DUTY COTBED [GOP-C-2022-096]

Dear SCMS,

Greetings!

We are formally writing in relation to the findings of our delivered goods, the Heavy-Duty Cot
Bed, under Contract No. GOP-C-2022-096.

On 27 October 2022, we were informed by the Health Emergency Management Bureau [HEMB],
the End – User of the delivered goods, that we need to pull – out the Total Quantity of the Heavy-Duty
Cot to address the concern [s] raised during the inspection. We were advised to coordinate with the
SCMS, and eventually, on 07 November 2022, we had received the letter from SCMS re the Pull-out of
the rejected commodity.

Given the circumstances, we respectfully request for your kind assistance in pulling out the
delivered goods from the Warehouse, so we can immediately facilitate and address the concerns of the
Inspection Team.
