In our first met, I introduced myself as a Nursing Student, and asked for her consent to have
an interview with her and to her family members. Then she gave me permission to conduct an
interview with her.
I asked for her Identification information, how many family members they have, what is their
source, who among them has a history of illness, how many times they eat in one day, and
what are their physical activities daily. Included here about their house and environment
sanitation as well as the hygiene route.
Through this Family Assessment form above I identified and indicated their family health
problem.
After our first interview with Miss E.R., I gave her some objectives and nursing Interventions
that she can apply to their family. After the interview with Ms. E.R., she allowed me to revisit
her on the next home visit.
Second Home Visit: (Date) October 29, 2022,

During my next home visit to the father, the head of the family, I asked him about the family health
problems objectives and intervention that I was given to him if there was any improvement. Based on his
story, they did general cleaning and disinfected the entire house and yard. They replaced the frenulum and
threw away those unnecessary things inside and outside the house. His other daughter and son also got
work including Miss Ramasasan for an additional source of income for their expenses at home. He thanked
me because the objectives and nursing intervention I taught them helped. Also the proper physical activities.

Third Home Visit: (Date) October 31, 2022,

The goal is partially met. The family now has proper sanitation, a good source of income to
support their essential needs, and proper exercise twice a week.
