3. Fullbody Harrness lanyard 100% hook up during movement at heights should be strictly
enforced. (Refresher safety training REQUIRED to all operatives assigned to work at heights)

4. Safety Inspectors to lead by example by wearing FBH lanyards properly (not shortened by tying)
