understand that as the Excavation Supervisor, it is my duties and responsibilities to ensure that the
provisions under the Workplace Safety & Health Act and any other statutory legislation requirements are
complied with.

My duties and responsibilities as an Excavation Supervisor are as follows:

1. Possess minimum Building Construction Supervisor Safety (BCSS) Certificate or higher.

2. Supervise the excavation, shoring and back filling of the Excavation under his charge.
3. Ensure all excavation works are in accordance to the requirement of the Reg. 77(2) to Reg. 77(11)
and Reg. 79 of the Workplace Safety and Health (Construction) Regulation.
4. Ensure all Excavations (> 4meters) are erected according with the design of Professional
Engineer, as require under the Workplace Safety and Health (Construction) Regulation.
5. Ensure that all shoring materials used are of correct type, adequate strength and of sound
condition.
6. Thoroughly check the Excavation and ensure it is safe before any entry is allowed
7. Immediately inform Project Manager if the excavation is in an unsafe condition.
8. Ensure all unsafe condition discovered during the inspections are remedied immediately before
allow entry into excavation.
9. Ensure that area of excavation is always being barricade and proper warning signages are being
put up.
10. Assign a watchman to ensure no person entering excavation when an excavation activity is in
progress.

I, hereby undertake to abide to the above-mentioned conditions of my appointment and fully comprehend
with the importance of my roles and duties as an appointed competent person/s. I shall take all necessary
measures to safely carry out the assigned site operations.
