understand that as the Excavator Operator, it is my duties and responsibilities to ensure that the
provisions under the Workplace Safety & Health Act and any other statutory legislation requirements are
complied with.

My duties and responsibilities as an Excavator Operator are as follows:

1. Possess minimum of an excavator operation certificate by authorized training provider

2. Possess a valid power grid license for working near the utilities.
3. Shall comply & follow strictly on the Risk Assessment, Safe Work Procedures, Work Method
Statement which have been endorsed and approved to follow by both LCAL & PUB;
4. Shall ensure that the Permit to Dig is being obtained and valid to start the daily work activities;
5. Shall ensure I am being brief by the immediate supervisor and fully understand types of underground
services in and near the working boundaries;
6. Shall ensure the control measures are in place to protect the underground services from being
damage by the work operation;
7. Shall ascertain whether the ground conditions, in particular the ground surface on which a crane is
to be operated, are safe for any boring operation, and if I am in the opinion that it is not safe for
any excavation operation, I shall report this to the Lifting Supervisor;
8. Shall ensure that the PTW for Excavation is being endorsed and approved before start work;
9. Shall ensure that the appointed Signalman/Banksman is deployed and I shall follow his signal
during the operation;
10. Shall ensure Take 5 and Hazard Identification form (LCAL) being put up before start;
11. Shall comply & follow the manufacturer’s Operational Manual & Maintenance for operating the
machinery;
12. Shall report any failure or malfunction of the Excavator to the lifting supervisor and shall make an
appropriate entry in the log book or log sheet of the excavator;
13. Shall not operate the excavator within (3) meters of any live overhead power line.
14. Shall only allowed to carry out light lifting operation with Excavator certify with AE as LM machines
with understanding of the SWL and the interpreting the load chart provided by manufacturer.
15. Shall ensure Lifting Supervisor & Rigger/signalman must be present for any light lifting operation to
be carried out using excavator with LM valid Cert.
16. Shall ensure and follow guides from the signalman deploy to assist and involve in the travelling of
the excavator.
17. Shall never use the excavator to perform any other task unless it is being certified by AE and to
seek clearance from both LCAL & PUB.
